- This is a combined synopsis/solicitation for commercial services prepared in accordance to the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. This is a Request for Quotations and a written solicitation will not be used. This solicitation document incorporates all applicable provisions and clauses in effect through the Federal Acquisition Circular 97-21. This procurement is 100% set aside for small business. The applicable (NAICS) Code is 562910 and the size standard shall not exceed $11.5 million annually. Clauses and provisions in effect for this solicitation are: FAR 52.212-1, Instruction to Offerors-Commercial Items;52.212-2, Evaluation-Commercial Items; 52.212-3, Offeror Representation and Certification-Commercial Items; 52.212-4, Terms and Conditions-Commercial Items; 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items. Copies of all of the above mentioned clauses are available on line at www.arnet.gov./far. FAR Clause 52.212-3, Offeror Representation and Certifications must be submitted with quotation. The Government intends to Award a firm-fixed price contract to the responsible offeror whose quote, conforming to this solicitation will be the most advantageous to the Government. Offeror will be evaluated on qualifications, past performance and price. Responses should clearly document those factors. Offeror shall submit pricing on emergency and non-emergency asbestos abatement. When contractor is notified for an non-emergency abatement, he/she will be notified at least five (5) to ten (10) days prior to needed abatement. When contractor is notified for emergency, he/she will be notified at any time and must respond at a maximum of twenty four (24) hours after VAMC notification. Offeror shall present emergency and non-emergency fees for the following services: Decontamination area (each); Poly enclosure (SF); Background and Sample Clearance (each); Removal of top layer of asbestos floor tile and base including mastic (SF); Removal of carpeting (SY); Removal of pipe insulation for pipes less than 3" in diameter by glovebag (LF); Removal of pipe insulation 3"-8" in diameter, inclusive by glovebag (LF); Removal of pipe insulation greater than 8" diameter by bulk (LF). Remove asbestos and 3" layer of contaminated soil (SF). Remove HVAC duct insulation (SF) ; Remove insulation from tanks, boilers and breeches (SF); Remove pressed asbestos board, siding, shield, ceiling insulation, paper insulation (SF); Remove sheetrock including joint compound and tape. Remove mechanical flex ducts, dampers and expansion joints. Pricing shall be submitted for Base Year: February 15, 2001 September 30, 2001 and Option Year 1: October 1, 2001 through September 30, 2002. Pricing shall be in accordance with current Davis-Bacon Wage Rates. Offeror shall provide the VAMC with emergency paging numbers and provide VAMC a maximum response time of 24 hours when emergency asbestos removal is required. Offeror shall provide certified asbestos workers experienced in abatement techniques, provide all necessary air onitoring,testing and documentation associated with asbestos removal. Contractor will be required to work at both facilities when called upon, (Brecksville Unit) 10000 Brecksville Road, Brecksville, Ohio 44141 and (Wade Park Unit) 10701 East Boulevard, Cleveland, Ohio 44106. Contractor shall submit copy of current licensure of the State of Ohio with their quote. Contractor shall be responsible for damage to any equipment and or systems that result from asbestos removal. All work shall be in strict accordance with all applicable Federal, State and local regulations, standards and codes governing asbestos abatement and any other trade work accomplished in conjunction with the abatement. Contractor shall submit pricing on all or none of the solicitation to include base year and option year renewal. For the sake of preparing quote, offeror shall base emergency and non-emergency hourly rate on a estimated 650 man hours for the Base Year and Option Year 1. Faxed Quotes will not be accepted.
