Description
This procurement is hereby issued for commercial items to be used by the VA and is prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12, Acquisition of Commercial Items, Subpart 12.6 and Part 13, Simplified Acquisition Procedures as supplemented with additional information included in this notice. Simplified acquisition procedures apply and the VA will conduct the procurement according to a combination of Parts 12 and 13. Solicitations are being requested under Public Law 104-262 The Veterans Health Care Eligibility Reform Act of 1996 and Title 38 United States Code Section 8153, with a Commercial Items format of terms, conditions, and provisions/clauses. This procurement is being conducted under Federal Acquisition Regulation (FAR) Subpart 13.5 test program. The NAICS Code for this procurement is 621999 and the gross annual receipts NTE $5.0 million. The Contractor shall provide to the VA Medical Center, Fayetteville, Arkansas, the services of a Board Certified physician(s) to perform the requirements of this contract. This contract is primarily for a Internal Medicine/Family Practice Physician (IM/FP), but, according to the needs of the medical center, one or more of the following specialties may also be required throughout the term of the contract: i.e., Anesthesia, Cardiology, Ophthalmology, Orthopedic, Psychiatry, Radiology, Urology. There may also be a need for a CRNA or Optometrist. Physicians may be utilized at the VAMC Fayetteville, the Gene Taylor Community Based Outpatient Clinic (CBOC) located in Mount Vernon, Missouri, and the CBOC located in Fort Smith. Services to be provided to the patients may include in-patient care, outpatient care, or both. Contract period is for one year with three one year options anticipated to begin May 1, 2001. This is to be a firm fixed price contract with an all inclusive hourly rate. This is 100% Set-Aside for Small Business. Physicians must be certified, licensed, and attain and hold an in-force Professional Malpractice Insurance in an amount not less than one million dollars ($1,000,000.00) during the contractual period along with other requirements as indicated in the solicitation package.
