Description

Provide radiopharmaceutical products to the Carl T. Hayden VA Medical Center for the period of 4/1/01 through 3/31/02 with the provision for four (4) one-year option periods. The radiopharmaceutical provider must be licensed by the Arizona State Board of Pharmacy "Pharmacy-Nuclear" and "Wholesale-Full Service." The radiopharmaceutical provider must hold a valid and current Radioactive Material License issued by the Arizona Radiation Regulatory Agency. All responsible sources may submit an offer which shall be considered. NAICS 325412 applies. All requests for the solicitation must be in writing or via FAX at 602-222-6467. Solicitation packages shall be available on or about 1/29/01.
