Description

The VA Northern Indiana Health Care System, Fort Wayne, Indiana has a requirement for Community Based Outpatient Clinic (CBOC) services to cover primary and preventive medical care and continuity of care to eleigible beneficiaries of the VA Northern Indiana Health Care System (VANIHCS). The eligibility of veterans will be determined by the VANIHCS and confirmed in writing to the provider. Veterans anticipated to receive primary care services reside in areas designated as Medically Underserved Areas/Medically Underserved Populations and/or Health Professional Shortage Areas located in St. Joseph, Elkart, Kosciusko, Noble, Marshall, Fulton, Lagrange, Pulaski, LaPorte, and Stark counties in Indiana, and Berrien, Van Buren, St. Joseph, and Cass counties in Michigan. Interested providers shall request the solicitation in writing. Requests are to be submitted on company letterhead to include telephone number, telefax number, point of contact and e-mail address if applicable. North American Industry Classification System (NAICS) code is 6213099 and has a small business size standard of $5.0 million average annual receipts for the last 3 years. The solicitation will be issued approximately January 25, 2001.
