- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-20. This procurement is unrestricted. The standard industrial classification (SIC) code is 8099, Health and Allied Services, N.E.C. The small business size standard is $5 million average annual receipts for the past three years. Offerors are encouraged to submit quotes based on an estimated 520 hours. Proposed rates should include all costs associated with providing the following services. Request For Proposals (RFP) 515-17-01, Interim Primary Radiologist. The VA Medical Center, Battle Creek, Michigan, has a requirement for interim coverage of a full time radiologist. Individuals proposed to work under this contract shall be board certified in diagnostic radiology with a full and unrestricted licensed in any State, Territory, or Commonwealth of the United States or the District of Columbia, and possess a current Basic Cardio Life Support Certification. Individuals proposed to work under the resulting contract will be required on-site 5 days per week, 8:00 a.m. to 4:30 p.m., from December 1, 2000, through February 28, 2001. A radiologist is required on-site no later than December 1, 2000. The contract radiologist shall primarily be required to interpret radiologic examinations, provide evaluation of patients, as requested; the response to be in the form of a dictated radiologic report. Reports shall be based on the interpretation of projections or radio images of the area to be examined, the selection of which is that of the interpreting radiologist. All services shall be performed "on site" at the VA Medical Centers in Battle Creek, Michigan. The contract radiologist shall be required to administer contrast agents or IV contrast with iodine, either directly to the gastrointestinal tract, or by some parenteral route. The contract radiologist shall be required to dictate the description of findings, and professional opinion into Computerized Patient Records System (CPRS) and shall be required to verify transcribed reports via electronic verification process. The contract physician will be required to observe all Medical Center policies and memorandums and Medical Center By-laws. Minimum Requirements: (1) Copy of full and unrestricted license to practice medicine in any State, Territory or Commonwealth of the United States or the District of Columbia; (2) Board certified in Neurology, (3) Specializing in Adult Neurology. Documentation of the minimum requirement stated above shall be submitted with offers. Evaluation criteria included below are, in descending order of importance, (1) Past Performance, (2)Competency, and (3) Price. Past Performance and Competency are significantly more important than Price. Offerors are required to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items ALT III (OCT 2000), with its offer.
