Description

The Phoenix Area Indian Health Service is requesting Dental Services for Indian Headstart children at seven (7) Phoenix Area IHS Service Units in Arizona. The contractor shall provide all necessary resources, personnel, facilities, equipment, transportation, property and supplies to deliver dental services in accordance with the acceptable standards of the American Dental Association's Council on Dental Therapeutics. Contract shall be from date of award for one (1) base year with four (4) option years. Closing date for receipt of proposals is December 20, 2000, 4:30 p.m., local time. Solicitation packages will be available November 20, 2000. Cite RFP 855.
