Description
Under the authority of 38 U.S.C. 8153 Expanded Sharing Authority and in accordance with FAR, Part 12 Acquisition of Commercial Items and FAR, Part 15, the Louis Stokes Cleveland, Veterans Affairs Medical Center, 10701 East Boulevard, Cleveland, Ohio, intends to procure services of 0.2 Full Time Equivalent Employee (FTEE) Primary Care Physician to provide history and physical examinations and vesting examinations. The contract physician shall be responsible for management of chronic medical conditions, preventive healthcare, triage of walk-in patients, initial stabilization of any urgent visits and submit referrals to the specialists at the Louis Stokes Cleveland, Department of Veterans Affairs Medical Center. The contract physician shall be on site at the East Liverpool Community Based Outpatient Clinic (CBOC)Cleveland, VAMC eight hours per week, excluding federal holiday. The exact day(s) of each week and the time will be mutually agreed upon between the Contractor and the Contracting Officer's Technical Representative (COTR. Services are required for base period from date of award through September 30, 2001 and two (2), one pre-priced option periods. This acquisition is a 100% small business set aside, the North American Industry Classification Code is 62211 and the Small Business Size Standard is $5.0 million. The following technical evaluation factors, listed in descending order of importance, shall be used to evaluate all proposals: 1. Technical: I. Adherence to Service Schedule comprised of (a) On-site Coverage; II Past Performance on Similar Contracts for the Past Three Years. III. Experience of Proposed Physician(s) in an academic setting. 2. Price Factors: I. Price Reasonableness. II. Price Relationship to State of Ohio Medicare Rates. Technical and past performance, when combined, are significantly more important than cost or price.
