Description
The USDA, Animal and Plant Health Inspection Service (APHIS), Veterinary Services (VS), Lincoln, NE, has a requirement for pickup and delivery service to various locations in Nebraska, and delivered to Topeka, Kansas, for the period December 1, 2000 through September 30, 2001. A fixed-price blanket purchase order. SIC 9999. Quotations ARE DUE NOVEMBER 15, 2000 by 10:00 a.m. local time. SUBMIT WRITTEN OR FAXED QUOTATION to "Purchasing Section," at (612) 370-2136. Simplified procurement procedures will be used as well as commercial item acquisition regulations per FAR 12. THIS IS A COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and A WRITTEN SOLICITATION WILL NOT BE ISSUED. This solicitation document incorporates provisions and clauses which are in effect through Federal Acquisition Circular 97-19. SERVICE REQUIREMENTS Daily pick up and delivery service by a bonded and insured courier is required to various locations in Nebraska, for the U.S, Department of Agriculture (USDA), Animal and Plant Health Inspection Service (APHIS), Veterinary Services (VS), Lincoln, NE, for the period of December 1, 2000 through September 30, 2001. Due to the nature of the program for which this service is required, it is impossible to accurately determine in advance or from past experience the number of pounds or the number of parcels that the services will be required for. However, it is estimated that the quantity shown will be required during the specified period. This estimate of needs is for information only and is not intended to imply that the estimate is an exact indication of the quantity that will be required. SPECIFICATIONS The USDA, APHIS, VS, Lincoln, NE, requires daily pickup service of parcels containing slaughter cattle blood samples Monday through Friday excluding Federal holidays as follows: 1. Pick up parcels daily between the hours of 5:00 p m and 7:00 p.m. at 3458 South 16th Street, Omaha, NE, 68108. Contact: Robert Stanek (402) 733-9426. Estimated 125 lbs per day, 2 ice chests, 1 mail bag per day. 2. Pick up parcels daily between the hours of 5:00 p m and 7:00 p m at Gibbon Packing Company, East Highway 30, Gibbon, NE 68840. Contact: Rhonda Dymek, (308) 468-5771. Estimated 125 lbs per day, 2 ice chests, 1 mail bag per day. 3. Pick up parcels daily between the hours of 5:00 p m and 7:00 p m at Lovett and Sons Pack, 1009 "M" Street, Hastings, NE 68901. Contact: Larry Butler, (402) 461-3412. Estimated 35 lbs per day, 1 ice chest, 1 mail bag per day. All parcels shall be delivered to USDA, APHIS, VS, State-Federal Laboratory, 1947 NW Topeka Boulevard, Suite E, Topeka, KS 66608, by 12:00 noon. the following day after pick up. All delivered samples are to be placed in the cooler (refrigerator) located inside the delivery door. Deliveries are to be made Tuesday through Saturday. Contact: Dr Kevin Varner (785) 235-2365. Upon delivery of blood samples, empty parcels are to be picked up at the lab and returned to the point of origin the following day. The Contractor shall furnish a clean and mechanically sound enclosed-type van or truck equipped to haul packages or containers. The Contractor shall furnish the vehicle with operator, and shall be responsible for all operating and maintenance costs of the vehicle. The drivers and vehicles shall be properly licensed to operate within the states of Nebraska and Kansas. The Contractor shall notify Robert D. Hanson (402) 434-2300 within one hour in the event of vehicle failure while transporting parcels. A back-up vehicle must be available to provide service within four (4) hours in the event of equipment failure. The Contractor shall receipt for all parcels picked up, such as bills of lading, airbills, packing lists, etc. The Contractor shall maintain a register of all parcels picked up and delivered, and obtain the signature of the destination agent. The Contractor shall be responsible for the loading and unloading of the vehicle. The Contractor shall enter the premises to the cooler area only. The Contractor shall invoice once per month, immediately after the close of each calendar month. Invoices shall be submitted in duplicate and show invoicing period, purchase order number, Taxpayer Identification Number, and invoiced amount. Invoices shall have applicable receipts attached as verification of charges. Invoices shall be submitted to: USDA, APHIS, VS, Attention: Robert D. Hanson, P. O. Box 81866, Lincoln, NE 68501. Quotes are to be based on cost per location, per day for pick up and delivery of parcels.
