Description

Provide medical radiology transcription services for the Carl T. Hayden VA Medical Center, Phoenix, AZ. Quotes shall be accepted only from bidders who are financially responsible and have established offices providing medical transcription services. Contractor must meet all requirements of Federal, State, and local codes and regulations. Term of the contract shall be from February 1, 2001 through January 31, 2002 with options to renew for four (4) additional one-year periods. Solicitation documents available on or about 11/13/00. This is a total small business aside. NAICSC code 561492 applies. All requests for copies of the solicitation shall be in writing. Fax requests shall be accepted at 602-222-6467. No telephone requests shall be honored. The place of performance is unknown. The Contracting Officer has requested a wage determination for the State of Arizona, the counties of Apache, Coconino, Gila, Maricopa, Navajo, Pinal and Yavapai Counties. The Contracting Officer shall request wage determinations for additional places of performance if asked to do so in writing. Additional requests for wage determinations must be received by the Contracting Officer by November 28, 2000 by 4:30 p.m. (local time). See note #1.
