- Description
- This notice is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Written quotes are being requested and a written solicitation will not be issued. This procurement is being conducted under Simplified Acquisition Procedures (SAP). The reference number for this procurement is RFQ 500-13-01 and incorporates the clauses and provisions that are in effect through Federal Acquisition Circular 97-14. Quotes should be for the following item: MIDCO BodyLoc Whole Body Stereotactic Localizer System or equal. The non-invasive system is intended for use as a precision patient immobilization and positioning system for the purpose of performing fractionated stereotactic radiotherapy/radiosurgery treatment. The System must be capable of immobilizing from the anterior and posterior for the head and neck, as well as, the thorax, abdomen, and pelvis. Its fuducial localized system must be continuous from head to pelvis and allow accurate and continuous stereotactic imaging and localization throughout the entire upper body. In addition, the localization fiducials must be embedded into the base and sides of the frame and can be seen on CT and MR imaging. The software program must provide for calculation of stereotactic coordinates from scanner images. Software must be functional on an IBM or IBM-compatible PC computer with Microsoft Windows 95. The frame must fit into most CT and MRI scanners having a bore diameter >55 cm and an image field of view of 48 cm. The precision rate for the CT Localization error in the transversal plane must be within .5 mm to 1 mm (not greater than 1 mm). The precision rate for the CT Localization error in the longitudinal plane must be with .4 mm to 2 mm (not greater than 3 mm). The precision rate for the LINAC Set-up error must be less than 9 mm. The walls of the body frame should be made of polycarbonate, a commonly used material in radiation therapy that does not cause artifacts on MR or CT images. Installation and Training should be included. Equipment to be delivered within 45-60 days after receipt of order. Contractor shall include warranty information. Quotes received shall specify FOB Destination. The provisions at FAR 52.212-1 Instruction to Offerors-Commercial Items and FAR 52.212-3 Offerors Representations and Certifications applies to this acquisition and a completed signed copy of this provision shall be submitted with the quote. The Representations and Certifications may be obtained via the Internet at: http://www.arnet.gov. The provisions at FAR 52.212-3 Evaluation-Commercial Items also applies to this acquisition. The clauses at FAR 52.212-4 Contract Terms and Conditions-Commercial Items and FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items applies to this acquisition. The following factors, in descending order of importance, will be used to evaluate quotes: (1) determination of product acceptability, (2) lowest priced quotations, and (3) delivery schedule and (4) technical ability.
