- This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. A formal solicitation WILL NOT be issued. Quotations are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ) and is being conducted under simplified acquisition procedures in accordance with FAR Part 13. The solicitation reference number is RFQ 544-024-01 and the incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-19. The NAICS Code is 621111 and the small business size standard is $5.0 million. This procurement is unrestricted. All quotations are due November 30, 2000 3:00 p.m. local time. Submit written quotations (oral or facsimile quotations will not be considered) to Chief A&FPS (90C), Attn.: Virgil Thompson, WJB Dorn VA Medical Center, 6439 Garners Ferry Road, Columbia, South Carolina 29209-1639. If hand carried, deliver to Chief A&FPS (90C), WJB Dorn VA Medical Center, Bldg. 22, Room 3B101, Columbia, South Carolina. Inquiries about this combined synopsis/solicitation should be forwarded by mail to the address above. Answers to questions of a substantive nature will be provided on the CBDnet as an amendment. This solicitation is for non-personal services. Quotations will be considered only from offerors who are regularly established in the business called for and who are financially responsible and have the necessary personnel to furnish service in the volume required for all the requirements under this contract. The government intends to award a firm-fixed price, requirements contract to the offeror or offerors whose quote conforms to the requirements of this solicitation, best meets the minimum needs of the government and has the lowest price, all factors being considered. The government may make multiple-awards to the responsible offerors in accordance with FAR 52.216-27. Award may be made without discussions, therefore offerors are encouraged to offer the best price at the time of submitting initial quotes. Successful offeror shall meet all requirements of Federal, State, County, or City codes regarding operations of this type of service. Supplies or Services and Prices/Costs: The WJB Dorn VA Medical Center intends to award a requirements contract for Physician Services to be performed by physicians who are Board Certified or Board Certifiable in Internal Medicine, or Family Medicine and are experienced in providing care in an outpatient setting. The requirement is for a minimum of one physician and a maximum of three physicians for a total estimated 6000 hours for a one year period for treatment of eligible veterans and other eligible beneficiaries located at the Department of Veterans Affairs Greenville Outpatient Clinic, Greenville, South Carolina. The contractor shall provide Physician Services in accordance with all terms, conditions, and provisions of the solicitation for a period of one year, starting immediately after credentials are established and appointment to the Medical Staff, estimated starting date is January 1, 2001 and ending December 31, 2001. Our Professional Standards Board must approve all physicians for appointment to the Medical Staff at the VA Greenville Outpatient Clinic prior to being assigned duties. The physicians shall perform the following functions at the facility: (1) Each Physician will evaluate and treat a minimum of 25 patients per duty day during normal working hours. If the workload exceeds 25 patients per physician, an hourly compensation at the same rate will be provided for the additional hours worked, not to exceed two additional hours per day per physician. If the physician fails to evaluate and treat the minimum of 25 patients per day, the hourly rate for that physician for that day may be reduced by 20%. The only exception is when the workload does not require the treatments of at least 25 patients. Physicians will remain on duty until all patients in the clinic have been treated and released. (2) The Contractor shall be responsible for providing a qualified replacement physician, with no break in coverage; in the event the scheduled physician is unable to report for duty. (3) Contractor's employees shall be present at the VA facility, actually performing the required services for the period specified in the contract, or the contract prices will be decreased accordingly during each billing cycle. (4) Contracted physicians will comply with JCAHO and Department of Veterans Affairs (DVA) requirements pertaining to patient treatment; the WJB Dorn VA Medical Center Quality Assurance Requirements; locally accepted tenets of standards of care; the 2000 Network Performance Plan VISN; Mission of the VA; and Medical Staff Bylaws. Non-compliance of these requirements may result in the revocation of clinical privileges. (5) Contractor shall provide a point of contact, available 24 hours a day, 7 days per week, including Holidays that can be contacted by phone or beeper, responsible for the contractor's performance under this contract. (6) Physicians providing services under this contract will comply with VA requirements for documentation of care and in the paper record and computerized medical record as available. Electronic entry and signing of orders is required as available. Appropriate documentation of services will be performed within established time frames using computerized (CPRS) or voice recognition. The contractor's employee(s) will be required to use the hospital computer system, to include but not limited to, checking electronic reports on a daily basis for accuracy and completeness, writing orders (i.e., Lab, consults and admission) approving patients results via electronic signature, and utilizing electronic mail to receive inquiries. (7) The VA pharmaceutical formulary will be used and will be strictly adhered to when prescribing medications. SPECIAL CONTRACT REQUIREMENTS: Under the authority of Public Law 104-262 and 38 U.S.C 8153, the contractor agrees to provide Health Care Resources in accordance with the terms and conditions stated herein, to furnish to and at the Department of Veterans Affairs Greenville Outpatient Clinic, the services and prices specified in this contract. 1. SERVICES: a. The services specified in the Sections entitled Schedule of Supplies/Services and Special Contract Requirements may be changed by written modification to this contract. The modification will be prepared by the VA Contracting Officer and, prior to becoming effective, shall be approved by the VA Under Secretary for Health or his/her designee. b. Other necessary personnel for the operation of the services contracted for at the VA will be provided by the VA at levels mutually agreed upon which are compatible with the safety of personnel and with quality research programming. c. The services to be performed by the contractor will be performed in accordance with VA policies and procedures and the regulations of the medical staff by laws of the VA facility. d. The services to be performed by the contractor will be monitored by the Clinic Manager or his designee. 2. TERM OF CONTRACT: This contract is effective for a period of one year after the starting date, estimated starting date is January 1, 2001 and ending December 31, 2001. This contract is subject to the availability of VA funds. The contractor shall perform no service after September 30, each year until the contracting officer authorizes such services in writing. 3. QUALIFICATIONS: Personnel assigned by the contractor to perform the services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. The qualifications of such personnel shall be subject to review by the VA Chief of Staff and approval by the VA Facility Director. b. Contractor will insure that all physicians to be employed under this contract participate in the credentialing and privileging process as described by the VA. No services will be provided by any contract physician prior to obtaining approval by the VA Chief of Staff and Director. The contractor will be provided copies of current requirements and updates as they are published. c. The VA reserves the right to refuse employment under this contract or require dismissal from contract work, of any contract employee whom, by reason of previous unsatisfactory performance at the VA or unsatisfactory performance. 4. TRAINING: a. Contractor employees shall attend or submit evidence of attendance at the following training: Safety, Infectious Control, EEO, Prevention of Sexual Harassment and Cultural Diversity. If training is provided by Dorn VAMC, classes will not be scheduled during an employee's scheduled tour of duty. Duties listed in the contract will not be canceled or delayed for a contract employee to attend any of the training listed above. 5. WORK HOURS: The services provided under this contract shall be furnished by the contractor as defined herein: a. Work Hours: Regular working hours is from 8:00 a.m. to 4:30 p.m., Monday through Friday, excluding legal holidays. b. National Holidays The Federal Government observes ten (10) days as National Holidays and these include New Year's Day, Washington's Birthday, Martin Luther King's Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving and Christmas and other days specifically declared by the President of the United States to be a national holiday. c. Non-work hours are hours from 4:30 p.m. to 8:00 a.m., Monday through Friday and 24 hours a day on Saturday and Sunday and national holidays. 6. PERSONNEL POLICY: The contractor shall be fully responsible for the protection of its personnel furnishing services under this contract, in accordance with the personnel policy of the contractor, such as providing worker's compensation, professional liability insurance, health examinations, income tax withholding, and social security payments. Personnel furnishing services under this contract will comply with annual infection control training, safety training and annual TB testing. c. The parties agree that the contractor, its employees, agents and subcontractors shall not be considered VA employees for any purpose. 7. RECORD KEEPING: The VA shall establish and maintain a record keeping system that will record work by the contractor's employee(s). 8. MONITORING PROCEDURES: The Contracting Officer will provide a written Delegation of Authority designating a Medical Center Staff to perform duties as the Contracting Officer's Representative (COR) to monitor contract performance. Monitoring will be conducted on a periodic basis. The Clinic Manager will be designated to perform COR duties under this contract. The Contractor will be provided a copy of the Delegation of Authority. The Clinic Manager shall monitor the work performed under this contract, using the following procedures: a. Monitors all evaluations and treatments completed under item 1, Addendum to Standard Form 1449 Block # 20 Supplies or Services and Price/Costs (Physician Services) by review of data entered into the DHCP Medical package. b. Reviews all information entered into the Risk Management database for results of patient follow-up. c. Changes in the terms, conditions, and/or provisions of this contract may be made only by written agreement of the parties. All changes are prepared and issued by the VA Contracting Officer. d. The Contractor may be required to remove any contract physician who is found not to practice within acceptable medical standards of care, violates patient care policies, or commits an unlawful act or act of patient/staff abuse including sexual harassment. e. Payment of contractor employees for any leave, including sick leave, holidays or vacations, is the responsibility of the contractor. 9. MEDICAL RECORDS: Physicians providing services under this contract will comply with VA requirements for documentation of care and in the paper record and computerized medical record as available. Appropriate dictation of services will be performed within established time frames.
