- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This procurement is conducted under simplified acquisition procedures in accordance with FAR Part 13. This announcement constitutes the only document to be issued. A formal solicitation WILL NOT be issued. This Notice incorporates provisions and clauses in effect through Federal Acquisition 97-19. RFQ 667-07-01 should be referenced on the quote. The Standard Classification (SIC) for this procurement is 8099 and the small business size standard is gross annual receipts NTE $5.0 million. This procurement is unrestricted. Requirement: The Overton Brooks VAMC, 510 East Stoner Ave., Shreveport, LA 71101, has an emergent need for a Board Certified or Board Elegilble (Board Elegible candidates can not exceed 4 years from the date they became elegible) Gastroenterologist Physician to provide GI and management services to veteran patients and eligible beneficiaries. Services shall be required 40 hours per week, Monday through Friday, 8:00 a.m. to 4:30 p.m., excluding Holidays. Overton Brooks VAMC, intends to solicit Quotes and award a firm, fixed price contract under the authority of Title 38 USC Section 8153. The contract is for a 12 months. The VA reserves the write to reduced the term of the contract with a 30 day notice if the recruitment efforts prove successful. Gastroenterologist shall be a citizen of the United States and shall hold a valid and unrestricted license to practice in a State, Territory, or Commonwealth of the United States or the District of Columbia and shall be proficient in the English language, both spoken and written. The Gastroenterologist shall perform the following functions at this facility: (1) Provide inpatient consultations in gastroenterology; (2) Conduct general hepatology and gastroenterology outpatient clinic services; (3) Be responsible for appropriate documentation of clinic notes, consultations, and procedure notes; (4) Perform routine/emergency diagnostic and therapeutic Endoscopy lab procedures, including but not limited to: Conscious sedation; Esophago-Gastroduodenscopy; Colonoscopy; ERCP; Liver biopsy; Paracentesis; and Percutaneous Endoscopic Gastrostomy tube placement; (5) Provide Gastroenterology on-call coverage as required. The clauses at FAR 52.212-4, Contract Terms and Conditions -- Commercial Items (May 1999) applies to this acquisition. There is an addenda to this clause. Add the following clauses: FAR 52.252-2, Clauses Incorporated by Reference (Feb 1998), This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. Award will be made to the offeror whose quote conforms to the requirements of the solicitation, best meets the minimum needs of the Government and has the lowest price, all factors being considered. Award made be made without discussions.
