Description

Whole blood and blood components for the Carl T. Hayden VA Medical Center, Phoenix, Arizona for the period of January 1, 2000 through December 31, 2001 with the provision for two (2) option years. Offeror must hold an unrevoked US License which is issued by the Director, Bureau of Biologics, FDA as a source of whole blood. Offeror must have a statement of approval regarding interstate shipment of blood that such approval has been authorized under Section 251 of the Public Health Service Act, as amended, 42 USC Section 262. All blood supplied to the VA shall be from volunteer donors in accordance with FDA rules and regulations. Routine blood and blood component orders shall be delivered as specified within 24 hours following oral or fax orders. ASAP's shall be delivered within 6 hours. With the exception of rare units, irradiated units shall be delivered within 3 hours of order and antigen negative units within 24 hours, or communication received indicating expected time frame of arrival due to rare source of antigen negative unit. Platelet antibody screens and crossmatches shall be completed within 24 hours of order. Stat serological problem cases shall be resolved within 12 hours; all other serological problem cases shall be resolved within 48 hours. Preliminary case resolution shall be communicated within 12 hours. Final resolution of each serological case is dependent on the case complexity. Stat orders of blood and blood components shall be delivered within one hour following phone order. Offeror shall have a distribution center for blood and blood components in the Phoenix metropolitan area. All responsible sources may submit offers which will be considered.
