Description
The Bureau of Engraving and Printing (BEP) is requesting quotations from vendors for a precision automatic custom index machine AT-5000 model manufactured by Precision Index Equipment Inc. or equal. The machine must adhere to all of the following salient characteristics: It must have tabbing as well as laminating capabilities; It must be able to form a plastic index tab made of Mylar and cut it to any length from 5/8 inches to 5-1/4 inches; Handles sheet sizes 5-1/4 inches X 8 inches wide up to 11 12 inches wide; After placing the index tab in the desired position, the machine must be capable of heatsealing (laminating) the Mylar to the sheet. Also, it must be able to perform the necessary final tab cutting followed by depositing the completed index sheet into the delivery hopper; It must reach production speed rates up to 5500 completed sheets per hour (with one operator); It must include one pair of standard 12 inch tab dies. These include replaceable heavy duty cutting blades which will give about 3,000,000 cuts; It must have a mylar sensor which detects any misfeed plastic to minimize waste; It must possess a solid state control system virtually eliminating electrical failure; It must have built-in static eliminators; It must have a non-marking friction feeder to allow continuous loading without stopping the machine; It must have presettable position counter for prevention of overrunning jobs; It must be able to work with virtually any brand tabbing film; It must be able to change both tab position and length of tab within two minutes; It must have a double sheet detector and be able to clear double-fed sheets in seconds; It must be able to run continuously while loading and unloading. The vendor also shall install the equipment in the designated location as set forth below. The simplified acquisition procedures set forth in FAR Parts 12 and 13 shall be used. This is a combined synopsis/solicitation. This notice constitutes the only solicitation; therefore, a written request for quotation will not be issued. This procurement is 100% set aside for small businesses (see numbered Note 1). The applicable NAICS (formerly SIC) code is 322222 and the associated small business size standard is 500 employees. The Request for Quotation (RFQ) number is 10-01-03. The total quantity is 1 unit. Delivery shall be made within 10 weeks after receipt of purchase order and installation shall be completed within one week from delivery. Delivery terms are F.O.B Destination within consignees premises. Delivery will be to the BEP Loading Dock at _D_ Street, Between 13th and 14th Sts. S.W., Washington, D.C. Room 411A Annex Building. The resulting purchase order shall be firm fixed price. Detailed technical information that demonstrates that all salient characteristics are fully met is required. Bidders may provide equipment literature and/or brochures for all pertinent information that will demonstrate the offered equipment meets the salient characteristics. Failure to provide complete information will render the bidder_s quote as unacceptable. If a bidder is providing an AT-5000, than technical information is not required. The basis for award shall be price, past performance, and delivery/installation schedule. All interested small businesses are requested to submit a written quotation which will be considered by the agency. Quotations may be submitted on a SF 1449 or on company letterhead. Quotations shall include the RFQ number, the proposed unit price, the proposed extended price, any prompt payment discount terms, company name, point of contact, address, phone number, fax number, tax identification number, DUNS number, and size status (i.e., small business, disadvantaged business or woman owned business). If this item is available under GSA Schedule please provide the schedule number. Quotations are due 10/26/00.
