Description
This procurement is unrestricted and all responsible sources may submit an offer. Provide materials and services to perform various chemistry, serology, hematology, microbiology, and immunity tests. The proposed contract is an Indefinite Delivery-Indefinite Quantity (IDIQ), requirements contract for the Naval Hospital Twentynine Palms, CA. However, the proposed contract will allow any DOD medical facility within TRICARE Region 9 to be added as an Ordering Activity. Accordingly, other commands may use this contract in the future. Courier services will be provided by the contractor. Contractor's laboratory must meet the following requirements for accreditation: 1) accredited by the College of American Pathologists, a state or regional reference laboratory, or licensed under the Clinical Laboratory Improvement Act of 1967. 2) Shall successfully participate in the college of American Pathologists comprehensive survey program. 3) Any approved subcontractors utilized must meet the above requirements. The contractor will have to provide a remote one-way teleprinter terminal or similar equipment or alternate mode to transmit test results. All services shall be rendered or supervised by individuals fully qualified and licensed as required by law. Period of performance shall commence on or about 27 Dec 2000, and will include a base period and four option years. Technical acceptability, price, and past performance will be used as evaluation factors for award. This acquisition will be conducted under North American Industry Classification System 621511, which establishes $5M threshold as the ceiling for small business size.
