Description

Under the authority of 38 U.S.C. 8153 Expanded Sharing Authority and in accordance with FAR, Part 12 Acquisition of Commercial Items the Louis Stokes Cleveland, Veterans Affairs Medical Center (VAMC), 10000 Brecksville Road, Brecksville, Ohio, intends to procure courier services. The Contractor shall pickup specimens from the Community Based Outpatient Clinics (CBOC) identified herein and deliver the specimens to the VAMC, Laboratory Service, located at 10701 East Boulevard, Cleveland, Ohio 44106. The pickup sites are as follows: (1) Youngstown CBOC, 2031 Belmont Avenue, Youngstown, Ohio 44505; (2) Mansfield CBOC, 1456 Park Avenue W, Mansfield, Ohio 44906; (3) Lorain CBOC, 205 West 20th Street, Lorain, Ohio 44052; (4) Painesville CBOC, 54 South State Street, Suite 204, Painesville, Ohio 44077; (5) New Philadelphia CBOC, 1260 Monroe Avenue New Philadelphia, Ohio 44663;(6) Warren CBOC, Riverside Square, 1400 Tod Avenue NW, Warren, Ohio 44485. Services are required for a base year and four (4) pre-priced option periods. The Contractor shall pickup the specimens between the hours of 3:30 P.M. and 5:30 P.M., Monday through Friday, excluding federal holidays. Delivery shall be made by 5:00 A.M. the next morning including federal holidays. Federal holidays include New Year's Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day, and any other day specifically designated by the President of the United States. This acquisition is 100% Small Business set-aside. The North American Industry Classification System code is 492110 and the size standard is 1500 employees.
