ADVERTISEMENT REQUEST FOR PROPOSAL RFP #00-023 HEALTH INSURANCE RFP #00-024 DENTAL INSURANCE RFP #00-025 LONG-TERM DISABILITY INSURANCE RFP #00-026 LIFE AND ACCIDENTAL DEATH & DISMEMBERMENT(AD&D) INSURANCE The St. Petersburg Housing Authority (SPHA), St. Petersburg, Florida, is requesting proposals from qualified, responsible firms interested in providing separate bids for 1) Health, 2) Dental, 3) Long-Term Disability, and 4) Life & AD&D Insurance to SPHA in accordance with the requirements of the U.S. Department of Housing and Urban Development ("HUD"). The following proposed schedule will apply to all four (4) solicitations. PROPOSED SCHEDULE October 8 & October 15, 2000 Advertise Request For Proposals October 9, 2000 Request For Proposal Documents ready for pick-up October 18, 2000 Pre-Proposal Conference A pre-proposal conference will be held at the St. Petersburg Housing Authority's Administrative Office located at 3250 5th Ave. N., St. Petersburg, Florida, 33713 at 2:00 p.m. Although not mandatory, it is strongly recommended that all interested Offerors attend. October 27, 2000 Proposals are due by 10:00 a.m. If you are interested in submitting a proposal for any or all of the above solicitations, please fax your request for a RFP package to (727) 893-1304 Monday through Friday, between the hours of 8:30 a.m. and 3:00 p.m. You may also pick-up a copy in person at 3250 5th Avenue North, St. Petersburg, Florida 33713 from Ms. Jackie Zilton during the same days and hours. SPHA RESERVES THE RIGHT TO REJECT ANY OR ALL PROPOSALS IF SUCH ACTION IS IN THE BEST INTEREST OF THE HOUSING AUTHORITY AND TO WAIVE ANY AND ALL INFORMALITIES AND MINOR IRREGULARITIES.
