Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR), Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and is being issued as a 100% small business set-aside, Standard Industrial Classification Code 8099 is applicable, size standard is $5 mil. Request for Quotations (RFQ) and a written solicitation will not be issued. The solicitation document and incorporated clauses and provisions are those in effect through Federal Acquisition Circular 97-19 and Veterans Affairs Acquisition Regulation VAAR, VA Acquisition Circular 97-8. The contracting activity located at the VA North Texas Health Care System (VANTHCS), 4500 S. Lancaster Rd. Dallas, TX 75216 anticipates making a contract award for certified Nuclear Medicine Technologist services for the period 10/21/2000 through 03/31/2001. Services shall include provision of two certified Nuclear Medicine Technologist to work 8 hours Monday through Friday, on call services of 1 technologist Monday through Friday from 4:30 PM to 7:30 AM, and 24 hour on call services of 1 technologist on weekends and holidays. The Contract shall also include four (4) six (6) month options allowing the government the unilateral right to extend the term of the contract for additional six- (6) months each. To include extensions the contract shall not exceed 30 months. STATEMENT OF WORK -- Work Hours: 8:00 AM to 4:30 PM, Monday through Friday excluding federal holidays. The 10 holidays observed by the federal government are: New Years Day; Washington's Birthday; Martin Luther King's Birthday; Memorial Day; Independence Day; Labor Day ; Columbus Day; Veteran's Day; Thanksgiving; and Christmas. For contract purpose any other day specifically declared by the President of the United States of America shall also be considered as a federal holiday. For the base period of the contract the normal work hours (8:00 AM to 4:30 PM) are estimated at 1,744. During the period there may be a need for the technologist to be called in on station. Payment for on call services shall only be made for the hours worked with a two-hour minimum payment being guaranteed. VA estimates there will be 225 to 250 paid hours required for the period. Option period hours shall be estimated at the time the option is exercised. Personnel Policy: The Contractor's personnel shall not be considered as VA employees. As appropriate, the Contractor shall be responsible for providing workers compensation, professional liability insurance, health examinations, income tax withholding, and social security payments. Monitoring Procedures: The mechanism for ensuring that contracted services are received shall be by the use of a sign in and out log located in Nuclear Medicine and maintained by the Chief Technician. Contract Administration: The VANTHCS activity that shall administer the contract is: Contracting Officer in Acquisition & Material Management (90C). The VA Contracting Officer reserves the right to designate a Contracting Officer's Technical Representative (COTR) for the purpose of supplying technical information with regard to the contract. Should said COTR be designated, the Contracting Officer shall provide a copy of the signed designation memorandum to the COTR and the Contractor. The memorandum shall list the specific authority and responsibilities of the COTR. The COTR shall be responsible for the certification of all invoices for contract services. Payments shall be made monthly in arrears upon receipt of a properly executed invoice. The invoice as a minimum shall contain all applicable information as cited in FAR clause 52.212-4(g), included by reference herein. Special Contract Requirements: All services specified above may only be changed by written modification to the contract. The services to be performed by the Contractor will be performed in accordance with VA policies and procedures and the regulations of the medical staff by-laws of the VA facility. The services to be performed by the Contractor shall be under the direction of the Chief of Nuclear Medicine who will verify that services provided are in compliance with professional procedure. Any extension of the contract shall be subject to the Contractor's past performance, the needs of the VA, and the availability of funds. Qualifications: Personnel assigned by the Contractor to perform the services covered by the contract shall be licensed in a State, Territory, or Commonwealth of the United States of America or the District of Columbia. The qualifications of such personnel shall also be subject to review by the VA Chief of Nuclear Medicine. FAR clauses and provisions incorporated herein by reference are FAR 52.212-1, 52.212-2, 52.212-3, 52.212-4, 52.212-5 and addenda: 52.217-9, 52.219-6, 52.233-2, 52.233-3, 52.237-7, 852.233-70 and 852.270-4). These clauses and provisions are available on the Internet web at http://www.arnet.gov/far. Those interested parties who are able to provide the requested services must submit their hourly rate quotations for both normal work hours as well as for on call services. Failure to provide both rates will be considered as non-responsive. Submitted quotations must include completed FAR Provision 52.212-3, "Offeror Representations and Certifications", and copies current license and provider qualifications. Offers must be received on or before10: 00 AM Dallas, TX local time on 10/17/2000. Offers must reference the solicitation number and be sent to the attention of the Contracting Officer.
