Description

This is a solicitation for commercial items prepared in accordance with the subparts in Federal Acquisition Regulation 12 and 13 and Veterans Administration Acquisition Regulations as supplemented with additional information included in this notice. A written solicitation will be issued. Solicitation number 673-47-01 applies and is issued as a request for quotations. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-19. The North American Industry Classification Systems (NAICS) code is 622110 @ $5 million. Provide EEG's, EMG's, and nerve conduction studies, for eligible veterans from the Department of Veterans Affairs (VA) Outpatient Clinic, 2900 Veterans Way, Viera, Florida, 32940, for a period of one year from date of contract award. Procedures shall be performed at contractor's location. Contractor's facility shall be located within a 25 mile radius of the VA Outpatient Clinic, Viera, Florida.
