- Note 22 -- Sole Source -- AGFA Corp THIS CONSTTIUTES A COMBINED SYNOPSIS NOTICE AND SOLICITATION. RESPONSE IS DUE NO LATER THAN 10 OCT 00. 0001 Service Maintenance Agreement (SMA) to support the Digital Imaging Network and Picture Archiving Communication System (DIN-PACS) within the Radiology Dept of the 78th Medical Group in accordance with below Statement of Work (SOW) 0002 SMA to support PIC equipment within the 78th Medical Group in accordance with below SOW. STATEMENT OF WORK This SMA will cover the hours of routing operations, 0800-1700 hours, and will include the following: Hardware Maintenance Services Include: Replacement Parts Labor charges, Travel & Expenses during SMA coverage hours Preventive Maintenance, inclusive of parts (applicable models) Technical Phone Support (24 hours per day, 7 days per week) Dial-in Remote Access Support (24 hours per day, 7 days per week) 30% discount on new IMPAX hardware upgrades Reduced after hours labor rate for on-site service Highest Priority On-Site Response Time during SMA coverage hours Hardware Upgrades are not included in the SMA. Agfa, however, will offer hardware upgrade programs as new technology comes into the market. SMA customers will be entitled to a discount towards IMPAX Hardware Upgrades. Some software releases may require a hardware upgrade. Software Maintenance Services Include: Dial-in Remote Access Support (24 hours per day, 7 days a week) Applicable Software Enhancements and Upgrades 30% Discount on new IMPAX/ADC Software Releases Technical Phone Support (24 hours per day, 7 days a week) Reduced after hours labor rate for on-site service Highest Priority On-site Response Time during SMA coverage hours On-site Intervention during SMA coverage hours New Software Releases are not included in the SMA. Agfa, however, does offer SMA customers a discount towards IMPAX/ADC Software Releases. Software Releases or "major software upgrades" are new software products released by Agfa licensed separately for a fee. A software release represents a change in the software basic design, structure, and functionality. Example: Release Series V3.0 to Release R4. Software Enhancements or "minor software upgrades" are modifications made to the current "Release" of software which enables software to perform new or different functions or increases the capability of the software or enables the software to function on new or different hardware or in a new or different software environment without changing its basic design, structure, or functionality. Example: Version 3.1 to Version 3.5.
