Description
The Veterans Affairs Medical Center located at 4815 N. Assembly St., Spokane, WA 99205 requires the service of Acute Dialysis for the period October 16, 2000, through September 30, 2001. The VA desires to arrange for the delivery of the technical components of Acute Dialysis services (the "Service") at the VA, together with appropriate administrative services which shall be provided by the contractor that employs a Medical Director and Nurse patient care providers specializing in the provision of Acute Dialysis services. The VA is purchasing new Dialysis equipment and when operational the technical and administrative components will be provided in their entirety at the VA Medical Center. Contractor shall provide patient care providers, which may include but are not limited to a Nurse Manager, staff nurses and/or technical assistants, who are duly credentialed and approved through the Allied Health Privileges mechanism of the VA. As a back-up system to the Veteran's Hospital dialysis machines and supplies, Contractor shall provide at its sole expense the equipment and disposable supplies specified in Exhibit A. It is further agreed and understood that Contractor shall obtain and provide, at its sole expense, all preventative maintenance and all emergency repair services to said equipment on a timely basis. Such maintenance service and the documentation thereof shall comply with the standards of the Joint Commission on Accreditation of Healthcare Organizations (hereinafter "JCAHO") and any other regulatory agencies or entities to which Contractor is or may become subject. Contractor shall provide Acute Dialysis services to inpatients of the Veterans Affairs Medical Center on the order of any physician credentialed in nephrology working for the Veterans Administration and, therefore, governed by the VA's Medical Staff Bylaws. Contractor Providers will be responsible for the care of the patient only as is directly related to the dialysis treatment and will assist the VA's staff in the other patient care needs only when appropriate and necessary. Contractor shall, as requested by the VA, advise with respect to the selection, procurement and utilization of supplies and support services required for the provision of the dialysis services not specifically included in this Agreement. Contractor shall, as requested by the VA, provide consulting on in-patient billing for dialysis services. Contractor shall assist the VA in preparation for accreditation surveys as they relate to the Service. Contractor shall participate in the VA's quality improvement program. Such participation shall include, but not be limited to, quarterly submission of reports and statistics to the VA's Medical Executive Committee. Contractor shall treat as confidential all information relating to the VA and departmental operations and shall not disclose such information to others in the absence of a duty to disclose or a right to receive such information. Contractor shall utilize appropriately licensed and/or certified Providers and shall provide the VA with documentation of Provider qualifications. The Providers shall apply for Allied Health privileges as provided for by the VA's Medical Staff Bylaws.
