Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulations document (FAR) subpart 12.6, as supplemented with additional information included in this notice and following the simplified acquisition procedures in FAR part 13.5. This announcement constitutes the only solicitation: proposals are being requested and A WRITTEN SOLICITATION WILL NOT BE ISSUED. Solicitation number 6TSA-00-JMT-0150 is being issued as a request for proposals (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 97-14. This procurement is for the following: CLIN 1: #MX48, 4 user port 8 channel, QTY. 26 total (Please Note 12 of this item will be delivered to Travis AFB, CA, 12 of this item will be delivered to McGuire AFB, NJ, & 2 of this item will be delivered to Scott AFB, IL.); CLIN 2: # BRKT1U, 19" Rack Mount Bracket for MX48, QTY. 26 total (Please Note 12 of this item will be delivered to Travis AFB, CA, 12 of this item will be delivered to McGuire AFB, NJ & 2 of this item will be delivered to Scott AFB, IL); CLIN 3: #CCP06U, 2' Cable kit for 1/PC/PS/2 or at computer, QTY. 192 total (Please note that 78 of this item will be delivered to Travis AFB, CA, 78 of this item will be delivered to McGuire AFB, NJ &36 will be delivered to Scott AFB, IL); CLIN 4: #CCP06F, 2' Cable for PS/2 KB, monitor & mouse, QTY 32 total (Please note that 13 of this item will be delivered to Travis AFB, CA, 13 of this item will be delivered to McGuire AFB, NJ & 6 of this item will be delivered to Scott AFB, IL); CLIN 5: #CCP40F, 13' Cable for PS/2 KB, monitor & mouse, QTY. 104 total (Please note that 48 of this item will be delivered to Travis AFB, CA, 48 of this item will be delivered to McGuire AFB, NJ, & 8 of this item will be delivered to Scott AFB, IL). The manufacturer is Raritan. Please include all requirements on quotations. No partial quotes. No substitutions. Indicate FOB destination on quote (shipping included in price). Please note the special shipping instructions with each CLIN # which includes multiple ship to addresses. The provision at FAR 52.212-1, Instructions to Offerors, applies to this acquisition. Evaluation is based on best value including past performance, cost and ability to meet service requirements. Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. The clause at FAR 52.212-4, Contract terms and Conditions-Commercial Items, applies to this acquisition. In addition to the guidance contained in FAR 52.212-4, contractors shall submit proof of delivery with their invoices. Proof of delivery may be in the form of a bill of lading or other shipping document signed by the receiving office. Invoices received without proof of delivery will not be accepted. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition.
