This Request for Quotation is to obtain government pricing for items that are not funded at this time, but have requested to receive funding this fiscal year through fall out funds, prior to 30 September 2000. Items purchased will depend on the amount of fall out funds. Therefore, the quotation should remain in effect until 30 September 2000. The requirement is book lease service for base library. Line item 0001: Adult book plan. Books in core collection of 2,100 with monthly book allowance of 105 books. The program shall provide the most current bestsellers and other just-published books, and shall furnish a large selection of titles from which the librarians can select monthly for an updated collection. The books shall be fully cataloged and processed before shipment. The library may send them back when they are no longer current and popular or buy them at a great discount. Performance period: September 2000 _ August 2001. (To start upon receipt of fund) Shipment of books shall be made to Yokota Air Base, APO AP 96328 as FOB destination. Line item 0002: Children_s hardback book plan. It includes over 750 new books added to the collection in one year, with at least 40 selected per month. The program shall provide current bestsellers for children. It is different from line items 0001 & 0003 because these books become a permanent part of collection. (Not to be returned) Performance period: September 2000 _ August 2001. (To start upon receipt of fund) Shipment of books shall be made to Yokota Air Base, APO AP 96328 as FOB destination. Line Item 0003: Young adult book plan. Including all cataloging and processing for a starter collection of 200 books and 60 books to be selected monthly by the librarians. Performance period: September 2000 _ August 2001. (To start upon receipt of fund) Shipment of books shall be made to Yokota Air Base, APO AP 96328 as FOB destination. The program shall provide the most current young adult bestsellers and other just-published books popular with Teens. The library may send them back when they are no longer current and popular or buy them at a great discount. The offerors shall include the following in their quotation: a. Unit prices (per month) and extended amount (12 months) for each of the three line items. b. (Summarized) program plan by fax or as e-mail attachment. c. Ordering name & address, e-mail address, telephone number, fax number, CAGE code, DUNS number, TIN, business size, and remittance/EFT address. Exclusion of any of the above may result in a delay in award and/or payment. To be eligible to receive award resulting from this quotation contractor must be registered in the Central Contractor Registration (CCR) database, NO EXCEPTIONS. In order to register, apply through the internet at http://www.ccr2000.com. A DUNS (Dun and Bradstreet) number is required to register. All responsible sources may submit a quotation, which shall be considered by the Agency.
