The Centers for Disease Control and Prevention (CDC) intends to purchase the following items (brand names required): 1.) 25 cases (12 Packs per case) of 3M #8210 L-994450 (95265), mask, respirators; 2.) 35 cases (2M per case) of Best #6005-PF L-994740 (94309), Glove, Size Medium, Nitrile, Powder-Free; 3.) 20 cases (2M per case) of Best #6005-PF L-982480 (99055), Glove, Size Small, Nitrile, Powder-Free; 4.) 35 cases (2M per case) of Best #6005-PF L-980350 (99095), Glove, Size Large, Nitrile, Powder-Free; 5.) 6 packs (12 pair per pack), #GL610PD L-904310 (95110), Glove, Size Small Men's Plas. DOT. All responsible sources that can meet the requirements listed herein may submit a quote which will be considered by the agency. This is a all or none, brand name requirement. Delivery must be made within 30 days of purchase. THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN SUBART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLITICTATION; PROPOSALS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. The following provision(s) apply to this acquisition: 52.212-1 Instructions to Offerors -- Commercial Items (MAR 2000) applies to this acquisition; 52.212-2, Evaluation -- Commercial Items (JAN 1999); the evaluation criteria stated in paragraph (a) of this provision are as follows: (1) technical capability of the proposed system to meet the requirements stated herein, and (2) price. The clause 52.212-4, Contract Terms & Conditions -- Commercial Items (MAY 1999) applies to this acquisition. The following FAR Clauses are hereby added to this clause: 52.247-35 F.O.B. Destination; The clause at 52.212-5 -- Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items applies to this acquisition. The following clauses listed in paragraph (b) of 52.212-5 apply to this acquisition: (1), (3), (5), (7), (9), (11), (12), (14), (15), (17), (18), (20) and (24).
