Description

This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-00-T-0104 is issued as a request for quotations(RFQ). The solicitation document and incorporated provisions and clauses are those in effect through FAC 97-19. This is a request for monthly recurring shipment of materials and reagents needed to perform Chlamydia and Gonorrhea testing with Gen-Probe instrument. Deliver to: Naval Medical Center, 34800 Bob Wilson Drive, Laboratory Department, San Diego CA 92134. CLIN 0001: Gen-Probe Pace 2 System, MFG/PN: Gen-Probe 1791, Quantity: 12 KIT; One (1) kit to be delivered the first week of each month from October 2000 through September 2001. CLIN 0002: Gen-Probe Pace 2 System, MFG/PN: Gen-Probe 1792. Quantity: 12 KT. One (1) kit to be delivered the first week of each month from October 2000 through September 2001. CLIN 0003: Gen-Probe Pace 2 System, MFG/PN: Gen-Probe 1793. Quantity: 12 KT. One (1) kit to be delivered the first week of each month from October 2000 through September 2001. CLIN 0004: Gen-Probe Pace 2C System, MFG/PN: Gen-Probe 3905. Quantity: 120 KT. Ten (10) kits to be delivered the first week of each month from October 2000 through September 2001. CLIN 0005: PCA for Chlamydia, MFG/PN: Gen-Probe 3548. Quantity: 12 KT. One (1) kit to be delivered the first week of each month from October 2000 through September 2001. CLIN 0006: PCA for Neisseria Gonorrhea, MFG/PN: Gen-Probe 3549. Quantity: 12 KT. One (1) kit to be delivered the first week of each month from October 2000 through September 2001. This acquisition incorporates the following FAR clauses: 52.212-1 Instructions to Offerors-Commercial Items (Oct95); 52.212-3 Offeror Representations and Certifications-Commercial Items (Oct95) must be completed and submitted along with quote.
