Ext. 300 The provision of Educational Diagnostic/Psychological
Counseling Services is required under PL 105-17, Indviduals with
Disabilities Education Act of 1997. Services consist of evaluation of
referred students in the area of educational diagnostic, making a
diagnostic statement as to nature of child's disability (if any)a
written report reflecting the findings of the evaluation, participation
in a multi-disciplinary team meeting regarding the evaluation results,
development of goals and objectives for those students identified as
needing special education services. Psychological counseling has also
been identified as a needed service at Nenahnezad Community School.
Services must be provided in accordance with PL 105-17 and the State of
New Mexico special education guidelines. Services will be provided for
the duration of the 2000-2001 school year. Services will start on
10/23/2000 and end 9/30/2001. Evaluations will occur in the order in
which the referrals were received by the Agency special education
office. Aneth Community School, Aneth, Utah with 23 Anticipated
Referrals, Nenahnezad Community School, Nenahnezad, NM with 45
Anticipated Referrals, Tiis Nasbas Community School, Teec Nos Pos, AZ
with 30 anticipated referrals and Beclabito Day School, Beclabito, NM
with 10 Anticipated Referrals. Nenahnezad Community School, Nenahnezad,
NM with 10 Anticpated Referrals for Psychological Counseling. It is
estimated that 60 days at 7 hours per day will be required to complete
the evaluations and that 40 day at 7 hours per day will be required to
conduct the psychological counseling. Additionally, payment is
authorized for the writing of reports and scoring of evaluations for
not more than 240 hours. The amount of required work could change
(increase or decrease) during the course of the school year depending
upon number of students referred for evaluation. Payment is also
authorized for mileage from the contractor's home to the school sites
and return. All school are located within a 50 mile radius of Shiprock,
NM. Contractor is responsible for all evaluation material and protocols
used. Services will be provide in the government owned property at
