provision of Occupational Therapy Services is required under PL 105-17,
Individuals with Disabilities Education Act of 1997. Services consist
of evaluation of referred students in the area of occupational therapy,
a written report reflecting the findings of the evluation,
participation in a multi-disciplinary team meeting regarding the
evaluation results, development of goals and objectives for those
studentsidentified as needing occupational therapy services and
provision of occupational therapy services for identified students.
Services must be provided in accordance with PL 105-17 and the State of
New Mexico special education guidelines. Some services can be provided
through the utilization of para-professionals employed at the school
site, having been provided work direction by the contractor. Services
will be provided for the duration of the 2000-2001 school year.
Services will start on October 23, 2000 and end on September 30, 2001.
Service will be provided at the school sites and the Agency Special
Education office for a total of 3 days per week plus one-half day (.5)
per week for paperwork. Schools/Location/# of Referred students/Days

Day School, Beclabito, New Mexico, 4 students and .50 days Tiis Nasbas
Community School, Teec Nos Pos, AZ, 12 students and 1.00 days Tohaali
Community School, Toadlena, NM, 9 students and 1.00 days Paperwork at
.50 days. The number of days per week could change (increase or
decrease) during the course of the school year, depending upon number
of students placed for occupational therapy services. Payment is
authorized for up to 4 hours (.5 day) per week for report writing and
scoring of evaluations. All schools are located within 50 mile radius
of Shiprock, NM. Contractor is responsible for furnishing
transportation, testing materials, protocols, and materials/equipment
to be used in provision of services. Contractor will be reimbursed for
