combined synopsis/solicitation for commercial service prepared in
accordance with the format in FAR Subpart 12.6 and 13.5 as
supplemented, with the additional information included in this notice.
This announcement constitutes the only solicitation. This is a Request
for Quotation and a written solicitation will not be issued.
Solicitation number is RFQ 523-3-01 and award will be made using the
simplified acquisition procedures. This solicitation document and
incorporated provisions and clauses are those in effect through Federal
Acquisition Circular 97-19. This acquisition is unrestricted. The
standard industrial classification (SIC) 7699, North American
Industrial Classification System (NAICS) 811219. The VA Boston
Healthcare System, 150 South Huntington Avenue, Boston, MA 02130-4893
intends to award a contract for preventative maintenance and repair of
one (1) Hitachi Clinical Chemical Analyzer Model 747-100. The
contractor is to furnish all supervision, personnel, equipment, labor,
parts, tools, materials, transportation and other items and services
necessary to perform maintenance on a Hitachi Clinical Chemical
Analyzer Model 747-100. Service shall include four (4) quarterly
preventative maintenance calls during the period of 1 October 2000 to
30 September 2001; Twenty-four (24) hour telephone technical assistance
service and guaranteed on-site response within twenty-four hours of
notification of a technical problem; Unlimited emergency service
(within 4 hours of notification) between the hours of 8:00 a.m. and
5:00 p.m. local time. The contractor shall also provide technical
assistance telephonically with suggested repairs in the case of an
emergency. PM's shall follow the original manufacturers' guidelines. by
service. The contractor shall document all repairs. The contractor
shall supply repair parts on an as-needed basis and conduct any
required repairs on site by routine or emergency maintenance. These
parts shall be billed on a not-to-exceed basis under the parts line
item of the issuedpurchase order. All workmanship on repairs and
maintenance shall be warranted for the term of one (1) year from the
date of acceptance by the government. All newly installed parts shall
be warranted for a period of one year or with manufacturer's warranty.
The Period of Performance is October 1, 2000 to September 30, 2001,
and the place of performance is the VA Medical Center, 150 South
Huntington Avenue, Boston, MA 02130-4893. Award is Subject to the
Availability of Funds. The provisions at 52.212-1, Instructions to
Offerors -- Commercial, applies to this acquisition. 52.212-2,
Evaluation -- Commercial Items applies to this acquisition with the
evaluation factors of past performance and price, which are weighed
equally and price must be considered fair and reasonable. The
Government reserves the right to award to other than the low offeror.
The offeror is to include a completed copy of the provision at
52.212-3, Offeror Representations and Certifications -- Commercial
Items, with its offer. The clause at 52.212-4, Contract Terms and
Conditions -- Commercial Items, applies to this acquisition. The clause
at 52.212-5, Contract Terms and Conditions Required To Implement
Statutes or Executive Orders -- Commercial Items, applies to this
acquisition. (1) TERM OF CONTRACT: This contract is effective for
twelve months from October 1, 2000 to September 30, 2001. (2) WORK
HOURS: Monday through Friday, 7:00 a.m. to 4:30 p.m. (3) NATIONAL
HOLIDAYS: The ten (10) holidays observed by the Federal Government,
i.e., New Years Day, President's Day, Martin Luther King Jr's Birthday,
Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day,
Thanksgiving Day, and Christmas Day. (3). QUALIFICATIONS: Personnel
under this contract shall be licensed or certified by the manufacturer
to perform services on the chemical analyzer. The qualifications of
such personnel shall also be subject to review by the VABHCS Chief
Biomedical Engineer and approved. (4). PAYMENTS: Sums due for actual
work done, the Contractor will be paid monthly, in arrears, upon
receipt of a properly prepared invoice FAR 52.232-25. The Contractor
shall submit all invoices to the following address: Chief Fiscal
Service (04) VA Boston Healthcare System, 150 South Huntington Avenue,
Boston, MA 02130-4893 (5) CONTRACT PERFORMANCE: a. The contractor will
be monitored through a variety of mechanisms, including but not limited
to -- sign-in/out log. -- Chart documenting repairs. b. The Chief of
Biomedical Engineering will be responsible for monitoring the quality
of services provided under this contract. (5) CLAUSES: The provisions
at FAR 52.212-1, Instructions of Offeror -- Commercial Items (Mar 2000)
applies to this acquisition. There is not an addendum to these
