accordance with Public Law 104-262 and 38 U.S.C. 8153. Contractor
agrees to provide an Adult or Family Nurse Practitioner, as a qualified
registered health care professional to and at the VA CBOC Bandon, 1010
1st. St. SE, Suite 100, Bandon, OR 97411. Nurse Practitioner must be
able to start on August 21, 2000. If candidate is not available to
start on August 21, 2000 offeror will be determined non-responsive, if
after award it is determined candidate cannot start on August 21, 2000
contract shall be terminated for cause and award shall be made to next
lowest offer. The Adult or Family Nurse Practitioner would be
responsible for a panel of approximately 400 patients whom he/she would
follow throughout the entire continuum of care. The provider will see
an average of 10-14 patients per day in the clinic. The ANP or FNP
would serve as the primary care provider to outpatients in the clinic
from 8:00 a.m. to 4:30 p.m., five days per week. The Nurse Practitioner
will have responsibility for primary care management, and will make
decisions regarding hospitalization, patient transfer, etc. The Nurse
Practitioner must: a. Have an unrestricted license in a US state,
territory or b. US commonwealth; b. Speak English proficiently; c.
Provide evidence of demonstrated current clinical competence to perform
appropriate primary care services; d. Be a US citizen; e. Have a
professional degree and education applicable to medical staff members,
e.g., NP, FNP, ANP; f. Provide evidence of current professional
liability insurance; g. Evidence of current privileges (if applicable);
h. Names of most current employer(s) and two peer reference contacts;
i. Current copy of BCLS certification; j. Be a team player; k. Must be
computer literate, as VA Roseburg Healthcare System utilizes a
computerized patient medical record; l. Nationally certified required
and Oregon licensed preferred. m. Must have a minimum of 2 years of
experience working as a NP. Provider assigned by the contractor to
perform the services stated in this contract will be credentialed and
receive privileges by the VA. Provider assigned by the contractor shall
dictate concise and comprehensive reports utilizing VA telephone
dictation processes. The services to be performed by the Contractor
will be performed within the VA policies and procedures and the
regulations of the Medical Staff Bylaws of the VA Roseburg Healthcare
System. Contractor must provide the following documentation along with
the candidate's information: 1. Evidence of current TB surveillance
status. 2. Evidence of current Hepatitis B vaccination and titer. 3. If
the contract physician is required to work in an environment that
requires a respirator, the contractor will either provide their
physician with a properly fitted respirator or the contract physician
will be required to go through VA Roseburg's Respiratory Fit Program
prior to beginning their assignment. Solicitation packages will be
