SUBSISTENCE DELIVERY SERVICES Blanket Purchase Agreement for the
Illinois Army National Guard. Under the Blanket Purchase Agreement
(BPA), the selected vendor will provide and deliver subsistence items
to 50 guard units on a monthly basis in three regions throughout the
State of Illinois. The base period of agreement is 1 October 2000 to 30
September 2001, with options for renewal for the next four fiscal years
(Oct-Sep). The estimated extent of the Government's Obligation is the
aggregate total amount of all orders placed under the BPA and shall not
exceed $210,000 per each period of the BPA (annually Oct-Sep of each
year). Using a Subsistence Automated Program, the ordering and payment
procedures will be supported via electronic order calls and payment by
International Merchant Purchasing Account Card (IMPAC) payments. The
vendor will be requested to deliver various food items as ordered on an
itemized subsistence worksheet. Food item categories include, meat,
saladdressings, bread, fresh produce, dairy, cereals, spices, desserts
and mixes, frozen vegetables, beverage products, soups, canned
vegetables, box lunch items. Prices will be set for the subsistence
items in the BPA and can be adjusted with modifications to the
agreement. One or more vendors may be selected for BPA(s) based upon
submission of the following data and information about the capabilities
of the vendor. To be considered for evaluation for the BPA, vendors
shall provide (1) an Evaluation Factor Questionnaire that lists the
vendor's ability to perform under the agreement; (2) Subsistence Price
List identifying the delivered price, distributed price and unit cost
of each item; (3) References-minimum of three references with which
your firm has provided like items. References shall contain length of
service provided, number of deliveries per month, number of locations
delivered to, number of monthly deliveries, total monthly value of
deliveries (ranges: $2,500-$10,000; $10,001-$25,000; $25,001-$50,000,
$50,000+). SIC 5141, Food Brokers/Groceries, General line Wholesale and
