synopsis/solicitation for commercial items prepared in accordance with
the format in Subpart 12.6, as supplemented with additional
information included in this notice. The announcement constitutes the
only solicitation; proposals are being requested and a written
solicitation will not be issued. (ii) This solicitation,
030-J34-0-0041, is issued as a request for quotation (RFQ). (iii) This
solicitation document and incorporated provisions and clauses are
those in effect through Federal Acquisition Circular 97-18. (iv)
Contractor will provide the development, distribution, of the SPARK
P.E. curricula and materials to the teachers and identified staff of
the Navajo Area Indian Health Service HPDP. Contractor will also
provide staff development based on agreed date and locations. Gallup,
New Mexico August 2 & 3, 2000; Fort Defiance, Arizona August 17 & 18,
2000; Kayenta, Arizona September 12 & 13, 2000; Crownpoint, New Mexico
September 13 & 14, 2000. (vi) [A] Task: 1.) Planning, developing,
organizing, and evaluating the implementation of the SPARK program on
the Navajo Nation. 2.) Contractor will provide the following
materials/services: ~a~. Curricula and Materials K-2 and 3-6 physical
education notebooks: Each teacher would receive the latest edition of
SPARK binders: over 300 pages and 30 sections containing reference and
resource chapters, instructional units, daily lesson plans, blackline
copy masters, and much more. A complete, practical document that
teachers will use the rest of their careers. (120) Notebooks for K-2 ~
(120) Notebooks for 3-6: Handouts: Includes instructional materials,
papers, cards, supplementary supplies, and prizes provided at each of
the inservice workshops. (120) Handouts for K-2: (120) Handouts for
3-6: ~b~. Staff Development: (Teachers of grades K-2): participate in
two full days of workshops during the fall of 2000-2001. A minimum of
12 hours of staff development is required. Cost for all the workshops
throughout the year includes: an estimated 12 hours preparation time,
2 hours for set-up and take down at the site each visit, 12+ hours of
active instruction, and salary for one trainer. (Teachers of grades
3-5): participate in two full days of workshops during the fall of
2000-2001. A minimum of 12 hours of staff development is required. Cost
for all the workshops throughout the year includes: an estimated 12
hours preparation time, 2 hours for set-up and take down at the site
each visit, 12+ hours of active instruction, and salary for one
trainer. ~c~. Follow-up Consultation/Facilitator
Training/Institutionalization: Contractor trains a teacher(s) at each
school to provide the follow-up assistance: The Contractor trainer will
conduct meetings/trainings of the school identified "on-site
facilitator(s)" after each in-service workshop. This teacher(s) would
work with Contractor as our liaison in promoting quality physical
education instruction and program continuity with their staff after the
intervention concludes. The goal being physicaleducation that lasts.
Facilitator training sessions include use of video tape; facilitators
are presented with materials and a folder, as well as a computer disc
(containing important documents for the facilitator to access). The
facilitator would then perform duties and responsibilities such as
prepping equipment, handling scheduling issues, model teaching and/or
assisting classroom teachers with lessons. Additionally, the Contractor
team would provide year-round supervision and availability for all
teachers and administration, not just the facilitator(s) via our 800
number and e-mail site. ~d~. Trainer of Trainers: Last year, we began
an extensive effort to ready teachers from the Navajo Nation to conduct
SPARK inservices. SPARK provided thorough training, materials, and
ongoing supervision for all participants in the T of T model. Many
Navajo Nation SPARK trainers have improved their presentation skills
and are almost ready to provide SPARK workshops on their own. This
year, we are proposing having these novice trainers co-present with
Certified and experienced SPARK trainers in all four geographical areas
referenced in this intervention. Contractor would coordinate their
presence at each of the workshops, "coach" them through the appropriate
agendas and staff development scenarios, and consult with them at the
conclusion of their teaching. Furthermore, we will continue to
individually prescribe ways of improving their selected content,
instruction, and follow-up. Contractor will also work with each area to
problem solve policy issues such as allocating dollars to continue to
pay the trainers for conducting SPARK workshops. Additionally,
Contractor's personnel will always be available via phone or e-mail to
support these trainers throughout the course of the year, after the
workshops conclude. ~e~. Navajo Trainer of Trainers Follow-up Meeting:
One day meeting, to be held in Chinle, AZ is to provide support and
continued training based on the model described below that was
conducted throughout the 1999-2000 school year. The Trainer of Trainers
model is designed to allow the Northern Navajo Medical Center it's own
"in-house" trainers who will eventually lead SPARK training sessions
across the Reservation. Individuals who have undergone the initial
SPARK training, have instructed the lessons for a minimum of one year,
and have exhibited enthusiasm and exceptional presentation skills are
eligible to undergo this training. Potential trainers will attend a
one day training session (grades K-2 or grades 3-6) in Gallup, NM.
Training will include: effective presentation techniques, organizing
and prepping inservice materials, video tape analyses, dealing with all
issues and aspects of the SPARK program including motivational
techniques, on-site equipment and scheduling issues, administrative
support, and student behavioral concerns. Materials provided: master of
task lists, agendas, overheads, handouts (research and newspaper
articles), inservice evaluations, facilitator folders, administrator
folders. Each service unit will house a set of instructional video
tapes and a computer disk (with task lists and agendas) that trainers
can check out as needed. Each trainer will also receive a "care
package" to include: SPARK letterhead, envelopes and "trinkets"
(pencils, key chains) which can be ordered from SPARK in the future to
replenish. Trainers will also receive a SPARK t-shirt. Contractor
personnel will always be available via phone or e-mail to support these
trainers. ~f~. Reporting Requirements: The contractor shall provide
summary of training sessions, number of participants, participants'
profession, number of school participating, etc. ~g~. Schedule: Upon
award of contract, contractor will initiate project and carry it out
during the two (2) months. Place of delivery of invoice for entire
project is: Office of Community Health Services, Northern Navajo
Medical Center, Shiprock, New Mexico 87420. Method of delivery should
be in person, by mail, or federal express. (vii) Government proposed
delivery date is August, 2000 through September 2000. FOB for this RFQ
is Destination. Performance of services shall be accomplished between
7:30AM and 4:30PM MDST Monday through Friday. (viii) Offeror must
comply with all instructions contained in FAR 52.212-1, Instructions to
Offerors-Commercial. (ix) The following additional FAR provisions and
clauses apply: FAR 52.212-2, Evaluation-Commercial Items, The
Government will award a contract resulting from this solicitation to
the responsible offeror whose offer is conforming to the solicitation
will be most advantageous to the Government, price and other factors
considered. The following factors shall be used to evaluate offerors:
1.) Ability to implement community running / fitness camps at the local
community level -- (25 points): 0 points no previous fitness camp
experience 25 points, 25 points -- Extensive experience in implementing
local, community-based fitness and running camps for Navajo children.
2.) Ability to provide services on-site in Navajo communities (15
points): 0 points no previous experience working with Navajo
communities, 15 points able to provide service on site at all 12 Navajo
communities in a timely fashion. 3.) Demonstrated experience in Native
American youth leadership training and ability to recruit, train, and
employ local Navajo and Native American role models and athletes as
instructors / facilitators / assistants for community fitness/running
camps (20 points): 0 points No experience in working with Navajo or
Native American athletes as employees or instructors, 20 points
Extensive experience in working with Navajo or Native American athletes
as employees and instructors. 4.) Methodology and Approach -- 20
points: Soundness, feasibility, and practicality of the proposed
approach and work plan to provide the series of camps/clinics/ and
events, Recognition of and plan for overcoming the difficulties
involved in implementing the series of camps/clinics. 5.) Understanding
of the Project -- 20 points: Perception of the needs and purposes of
this project as demonstrated through a knowledge and understanding of
current health and fitness promotion efforts for native people, past
efforts, and the clarity and practicality of the offeror's proposed
implementation plans. Price will be evaluated to determine fairness,
completeness, and reasonableness as it relates to the services offered.
(x) FAR 52.212-3, Offeror Representation and Certifications-Commercial
Items, a completed copy of this provision shall be submitted with the
offer. The following provision applies: FAR 52.212-4, Contract Terms
and Conditions- Commercial Items . Offers are to be received no later
