R -- PATIENT DATA COLLECTION-INTERNET BASED SOL RFP 573-00-45 DUE 060200 POC Tami Drake or Susan Little, PH 904-755-3016, EXT 2096, FAX 904-758-3211 This is a notice of intent of the North Florida/South Georgia Veterans Health System, and the requirements for Veterans Administration sites located in Fort Myers, Bay Pines and Miami Florida to negotiate sole source with Health Hero Networks, Inc., to furnish an in-home appliance (not a computer)and on-line internet service for chronic disease management through internet based patient question dialogs for specific disease conditions. This procurement is being conducted under the authority of far 6.302 for a period of one year with a one year renewal option. The authority cited and justification is made based on the determination that the government's minimum needs can best be satisfied by services available from one source. Other persons may identify their interest and capability to respond to this requirement by June 2, 2000. Such qualifications and capabilities will be used solely for the purpose of determining whether or not to conduct this procurement on a competitive basis. This notice of intentis not a request for competitive proposals. However, all responses received on or before 6/2/00 will be considered by the government. A determination by the government not to open the requirement for to competition based on responses to this notice is solely within the discretion of the government.
