synopsis/solicitation for commercial item services prepared in
accordance with the format in FAR Subpart 12.6 as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation; written proposals are being
requested. A written solicitation will not be issued. This acquisition
is unrestricted. This document and incorporated provisions and clauses
are those in effect through FAC 99-17. SIC 8099 applies. Size Standard
is $5 million in average revenues over the last three years.
REQUIREMENTS -- 5 day per week coverage (40 hours), 8:00 a.m. to 4:30
p.m., with possible on-call services, evenings, and weekends: Item 1.
While VAMC recruits a Staff Radiologist, Contractor to provide Board
Certified, unrestricted license in U.S., Radiologist services to and at
VA Medical Center, Martinsburg, WV for period June 5, 2000 through July
31, 2000. Duties will include performance of radiologic procedures
appropriate to a Board Certified Radiologist. Item 2. Contractor to
provide Board Certified Psychiatrist for period May 30, 2000 through
July 31, 2000. Candidates having completed their residency program is
acceptable as long as the individual is within four years of completion
of residency. Individuals who are board eligible, but are beyond the
four-year limitation for board certification, will not be considered.
The Contractor shall provide worker's compensation, Professional
Liability Insurance, malpractice insurance, employee health/infection
control program and appropriate immunizations, income tax withholding
and social security payments for these personnel. The contractor, its
employees, agents and subcontractors shall not be considered VA
employees for any purpose. Contractor's employees are not covered by
the Federal Torts Claim Act. Contract shall invoice monthly in arrears.
Duties for both disciplines will include inpatient and outpatient
consult visits. Comply with appropriate and applicable policies,
procedures and regulations of the Martinsburg VA Medical Center. The

beneficiaries. All offers will be reviewed based on the following:
Professional qualifications, clinical and consultation skills,
availability, past performance and cost. Proposals shall include all
information required in 52.212-1, Schedule of Prices Offered.
Prospective contractors must submit a completed copy of provisions
52.212-3, Offeror Representations and Certifications-Commercial Items,
and 852.219-70 Veteran Owned Small Business Certification along with
pricing/technical quotation (hourly rate), documents necessary for
credentialing and privileging the provider-candidates, copies of
current licenses and qualifications with the offer. Contract
Practitioners may not begin seeing patients until all credentialing and
privileging requirements according to JCAHO requirements and VA
procedures have been met and necessary documents signed by the VA
Facility Director. The contract is through July 31, 2000 with two
one-month extensions. The government intends to evaluate offers and
award without discussions. However, the government reserves the right
to conduct discussions later if determined necessary by the Contracting
Officer. Solicitations are being requested under Public Law 104-262 The
Veterans Health Care Eligibility Reform Act of 1996 and Title 38 United
States Code Section 8153, with a Commercial Items format of terms,
conditions, and provisions/clauses. This procurement is being conducted
under Federal Acquisition Regulation (FAR) Subpart 13.5 test program;
simplified acquisition procedures apply. VA will conduct the
procurement according to a combination of FAR Part 12, Acquisition of
Commercial Items and Part 13, Simplified Acquisition Procedures.
