Contract will be based on an all inclusive capitated rate. Initial primary care visits will consist of physician's office visit including medical history and physical exam, clinical assessment diagnosis and treatment. Specialty consultation and more extensive diagnositc workups (e.g., Cardiology, Pulmonary, Dermatology, and Computerized Axial Tomography) shall be referred to the STVHCS. The contractor must be able to supply laboratory and radiology services. The clinic(s) are required to be geographically located within/near the areas they are placing proposals. The areas are: San Antonio (Bexar County), San Marcos (Hays County), Seguin (Guadalupe County), and New Braunfels (Comal County). Services are to be performed at the contractor's site.
