(541) 440-1331 Provide healthcare resources in accordance with Public
Law 104-262 and 38 U.S.C. 8153. Contractor agrees to provide a Board
Certified Physician in Urology, at the VA Roseburg Healthcare System,
Roseburg OR. Physician must be able to start on June 12,2000. If
candidate is not available to start on June 12, 2000 offeror will be
determined non-responsive, if after award it is determined candidate
cannot start on June 12, 2000 contract shall be terminated for cause
and award shall be made to next lowest offer. The Physician will see an
average of 12 patients per day in clinic, 4 consults each day and
performing 7 ambulatory surgical procedures each day. The Physician
will be assigned to Operative Care Service from 8:00 a.m. to 4:30 p.m.,
five days per week. The Physician will have responsibility for but not
limited to ordering diagnostic tests as indicated, prescribe
medications, perform ambulatory surgical procedures to include but not
limited to cystoscopies, urethral dilatations, transrectal needle
biopsies of prostate (ultrasound assistance with technician, with the
expectation that urologist will obtain biopsy specimens). Urologist
must be able to utilize flexible and rigid Storz instrumentation. The
Physician must: a. Have an unrestricted license in a US state,
territory or US commonwealth; b. Speak English proficiently; c. Provide
evidence of demonstrated current clinical competence to perform
appropriate outpatient care services; d. Be a US citizen; e. Be Board
Certified; f. Have a professional degree and education applicable to
medical staff members, e.g., MD, DO; g. Provide evidence of current
professional liability insurance; h. Evidence of current privileges (if
applicable); i. Names of most current employer(s) and two peer
reference contacts; j. Current copy of BLS certifications; k. Be a team
player; l. Must be computer literate, as VA Roseburg Healthcare System
utilizes a computerized patient medical record; Contractor must
provide the following documentation along with the candidate's
information: 1. Evidence of current TB surveillance status. 2. Evidence
of current Hepatitis B vaccination and titer. 3. If the contract
physician is required to work in an environment that requires a
respirator, the contractor will either provide their physician with a
properly fitted respirator or the contract physician will be required
to go through VA Roseburg's Respiratory Fit Program prior to beginning
their assignment. Solicitation packages will be available after
