commercial items prepared in accordance with the format in FAR Subpart
12.6 & FAR 13 as supplemented with additional information included in
this notice. This announcement constitutes the only solicitation.
Request for Quotes (RFQ) are being requested and a written solicitation
will not be issued. The solicitation number is RFQ #565-18 -00. The
solicitation document and incorporated provisions and clauses are those
in effect through Federal Acquisition Circular 97-14. The SIC code is
7389 and the small business size standard is $5.0M. The VA Medical
Center, Fayetteville, N.C. intends to solicit quotes and award a firm
fixed price contract on a competitive basis under the authority of
Title 38 USC Section 8153, to the offeror(s) whose quotation meets the
requirements of this solicitation. The VA reserves the right to award
more than one firm fixed price contract if it is determined to most
advantageous to the government. Quotes are due by close of business,
4:30 p.m. local time, April 14, 2000. The VA Medical Center
Fayetteville, NC has an immediate need for services of a commercial
recruiting firm to conduct a contingency search as an additional source
of applicants for the purpose of referral of a board certified/board
eligible Internist/Hospitalists (2 positions) with special interest,
training, or certification in Geriatric medicine for employment at the
VA Medical Center, Fayetteville, N.C Hospitalist Work Statement:
Physician will be required to provide acute care/observation bed care
to all medical acute care admissions, including ICU. Physician will be
responsible for complete assessments; diagnosis; daily provisions of
care and documentation; discharge planning; patient and family
education; communication arrangements for follow-up care; completion of
in-patient medical records; obtaining autopsy permission; obtaining
informed consent for medical procedures and processes or procedures
relevant to an in-patients care from admission to discharge. Physician
will be expected to cross over with Geriatrician Internist. The
average workload per physician is approximately 15-20 assigned
patients. A mid-level practitioner will assist each physician.
Physicians will be required to work nights, weekends, holidays, and
rotating shifts. Candidates must possess a current, full and
unrestricted license in a State, Territory, or Commonwealth of the
United States or the District of Columbia. Candidates must be board
certified or board eligible (physicians who are board-eligible in the
relevant specialty area and less than 4 years out from completing
his/her post graduate training) in Internal Medicine and must meet the
qualifications and credentialing/privileging requirements of the VA
Medical Center, Fayetteville, N.C. Candidates must be eligible to
practice as a bonafide physician within the United States or District
of Columbia. Candidates must be U.S. Citizen or permanent resident of
the United States. Contractor will be responsible for screening
candidates against the basic qualification requirements and verifying
licensure of all candidates before referring to Human Resources
Management Service at the VA Medical Center, Fayetteville, N.C. All
candidates meeting these requirements will be referred. Proof of such
licenses and board certification will be submitted upon request by the
VA. Candidates will be required to meet all requirements as set forth
in the statement of work. The salary range for hospitalists hired
within the VA system is from $65,983.00 to $100,897.00, salary
commensurate with education and experience. Benefits include: 30 days
paid vacation/15 days sick leave annually, ten paid holidays, excellent
retirement savings plan, and health & life insurance shared expense,
malpractice insurance furnished by the VA. Selected physicians are
required to sign an agreement to work for the VA for a period of two
years. If the Hospitalists do not fulfill his/her obligations a refund
of special pay will be paid to the VA by the physician. The contractor
will not issue any recruitment advertisements identifiable to the
Department of Veterans Affairs or the VA Medical Center, Fayetteville,
N.C. without the approval of the Contracting Officer. The Contracting
Officer reserves the right to review, rejects, or modify any
recruitment advertisements proposed by the commercial firm. All
advertisements must contain a statement that the Medical Center is an
EEO/MF opportunity employer. Contractor may not imply that it is the
sole or primary avenue for employment with the Department of Veterans
Affairs or the VA Medical Center, Fayetteville, N.C. Review and
approval of recruitment advertisements may be accomplished by FAX to
eliminate mail request. The FAX point of contact is Janet A. Hudson,

for requiring candidates to complete VA Form 10-2850, Application for
Physicians, Dentists, Podiatrists and Optometrists. Application will be
forwarded to the Contracting Officer at the VA Medical Center,
Fayetteville,N.C. once contractor has determined that candidates meets
all requirements of the specifications. Contractor will provide
physicians application form, Curriculum vitaes, current references and
summaries of all malpractice exposures to the Contracting Officer no
later than 30 days from date of award. If contractor fails to provide
qualified applicants within this timeframe, contract shall be
terminated for cause and no funds will be expended to the contractor,
award shall be made to the next lowest offer. Once recruited,
applicants will be evaluated and considered for appointment by the VA
through the Department of Veterans Affairs accepted employment
procedures. This includes reference and background checks, credentials
verification, application of veterans preference, etc. Oral interviews
will be conducted between the VA and the applicant prior to acceptance.
Candidate will be subject to successful completion of a pre-employment
physical examination and drug screening prior to employment.
Physicians employed by the VA as a result of this contract will work a
two-year probationary period. At the end of the two-year probationary
period, the physician will be reviewed by the VA Physician
Professional Standards Board. If acceptable level of performance has
been demonstrated, the physician will become a permanent employee of
the VA. Based on this requirement, the Contractor guarantees their
services for a period of two years from the entry on duty date of the
physician. In the event the physician leaves employment with the VA
Medical Center, Fayetteville, N.C. within the first year of the
probationary period, the contractor agrees to provide another
contingency search for a qualified Hospitalist at no cost to the VA. If
the physician leaves employment with the VA Medical Center,
Fayetteville, N.C. within the second year of the probationary period,
the contractor agrees to provide another contingency search for a
qualified Hospitalist in accordance with the terms of this solicitation
at a cost of one half the original fee. The VA is actively recruiting
for the position of Hospitalist through normal VA procedures. The
commercial recruiting firm will receive no fees for applicants who
apply directly to the VA Medical Center, Fayetteville, N.C. without
having been referred by the commercial recruiting firm. Applicants may
apply directly to the VA Medical Center, Fayetteville, N.C. and thus
need not apply through the commercial recruiting firm. Candidates who
have applied directly will be given the same consideration as those
referred from the commercial recruiting firm.. Fees shall not be
charged by the contractor to the individual candidates for referral for
the position of Hospitalist at the VA Medical Center, Fayetteville,
N.C. If the candidates have previously paid the commercial recruiting
firm any type of registration fees, membership fees, or recruiting
fees, the VA will not be responsible for any type fees due the
commercial recruiting firm by the candidate if the candidate is
accepted for employment by the VA. The contractor will refer only those
applicants from whom the firm or service has not accepted fees other
than registration type fees that are imposed regardless of whether the
registrant is referred for employment or placed Upon award of
contract, the VA will furnish to the contractor VA Form 10-2850s,
Application for Physicians, Dentists, Podiatrists, and Optometrists,
Fact Sheet on the VA Medical Center, Fayetteville, N.C. and other
pertinent information regarding employment with the VA. PAYMENT:
Contractor may provide multiple applicants, however, payment will only
be made if a successful applicant is chosen from those offered.
Recruitment services is for two positions of hospitalists. The VA will
pay the Contractor of the agreed fee upon acceptance of a hospitalists
by the Chief of Staff of the VA Medical Center, Fayetteville, N.C. and
upon signing by the physician of the commitment letter of employment.
The remaining fee balance will be paid at the completion of the
physician's 90-day evaluation. Pricing Schedule: 1. Commercial
recruiting service for the position of Hospitalist #1
$__________________ cost per position. 2. Commercial recruiting service
for position of Hospitalist #2 $_______________cost per position. Total
Cost for 2 Hospitalists $____________________. (Offeror does not have
to submit a quote for both positions to be considered responsive). The
following provisions apply to this solicitation: 52.212-1,

52.212-5, Contract Terms and Conditions required to Implement Statutes
or Executive Orders-Commercial, including clauses incorporated by
reference under Section b (1)(8)(11-16)(23-24); Award will be made to
the offeror whose offer conforming to the solicitation will be most
advantageous to the Government, price and other factors considered. The
government intends to evaluate offers and award without discussions.
However, the government reserves the right to conduct discussions later
if determined necessary by the Contracting Officer. The information
required in provisions 52.212-1, 52.212-2, a completed copy of
provisions 52.212-3, Offeror Representations and
Certifications-Commercial Items, and 852.219-70, Veteran Owned Small
Business certification must be submitted with offer. Quotes must be
received by close of business April 14, 2000 local time. Proposals
