The Contractor shall provide Basic Life Support (BLS), Advanced Life Support (ALS) and patient transport services and other government requested ambulance services for Winslow Indian Health Center (WIHC) Winslow, Arizona. The Contractor shall provide all necessary personnel, pharmaceutical drugs, medical supplies, equipment, vehicles, facilities and other items an expertise necessary for or incidental to provide all ambulance services. Services shall be available twenty-four hours per day, seven days per week. The Contractor must possess a valid Certificate of Necessity and License issued by the State of Arizona. Anticipated award will be for a twelve-month period from 01 MAR 2000 with two (2) one-year option.
