xvargas@mlca.uscg.mil. POTENTIAL SOURCE SOUGHT. The U. S. Coast Guard
Maintenance and Logistics Command Atlantic is looking for an individual
or firm capable of providing Psychiatric Social Worker services at the
Coast Guard Training Center Cape May New Jersey. This requirement is
for a full time position for a base and a four (4) one-year option
periods. The SIC Code for this acquisition is 8049 with $5 Million or
less average annual receipts for all businesses within the last three
(3) fiscal years. QUALIFICATIONS: The Psychiatric Social Worker shall
possess and maintain for the duration of this agreement the following:
1. A diploma from an accredited School of Social Work and a Masters
Degree in Social Work. 2. A current unrestricted license for the
practice of Social Work in one of the 50 United States. 3. A Current
certification in the field of Social Work. 4. Current certification in
Cardiopulmonary (CPR) by the American Red Cross or American Heart
Association. 5. A minimum of two (2) years recent (immediately
preceding the submission of this offer) clinical experience in
providing like services for the Coast Guard or other military, Federal,
or comparable civilian Psychiatric Social Work practices of the same or
similar scope and/or size for this service. 6. The Psychiatric Social
Worker must be able to meet requirements (imposed by the state in which
they are licensed) of collaborative (to include prescriptive authority)
for employment on a Federal installation/reservation. 7. A _no adverse
information on file_ with the National Practitioners Data Bank (NPDB).
PROFESSIONAL SERVICES REQUIRED: The provider shall perform a full range
of Psychiatric Social Work procedures on site using Government
furnished facilities, equipment and supplies. Caseload includes
scheduled and unscheduled requirements for care. The actual clinical
activity will be a function of the U. S. Coast Guard's credentialing
process and the overall demand for Psychiatric Social Work services.
All aspects of Psychiatric Social Worker service shall be commensurate
with the capabilities of the individual provider and the Medical
Department as follows: 1. Assess and treat outpatients in individual,
group, couples, and family modalities exercising mature professional
judgment and using a wide range of social work skills to include
individual and family counseling to assist patients and their families
in dealing with chronic and acute diseases/injuries. 2. Conduct
psychosocial assessments to determine patient needs and resources (both
family support and community support). 3. Provide counseling to patient
and family in matters directly related to patients limitation,
adjustment to medical condition, and/or ongoing treatment. 4. Develop
and implement discharge plans, follow-up care, and transfers to other
health care facilities (e.g., nursing homes, rehabilitation hospitals,
and VA facilities). 5. Plan and maintain referral and coordination
services with civilian health and social service agencies to provide
optimal patient care. 6. Provide consultation services to medical staff
regarding Psychosocial to insure, discharge plans, and follow-up care
for patients and families. 7. Provide crisis intervention services as
directed by the Medical Administrator Officer. No solicitation for
