Clemons, (562) 961-8112 VISN 22, Network Business Center (NBC) is currently seeking a highly qualified healthcare provider (sources sought only) to fulfill the level of medical care necessary for outpatient care and preventative health services at a capitated rate (a turn-key operation) to all veterans upon eligibility verification and enrollment at the selected primary care clinic facility. This facility shall be referred to as a, "Community Based Outpatient Clinic (CBOC)." The CBOC target area/location is Whittier, California. The healthcare provider shall provide clinical space, primary care services, and all pertinent professional services. The period of performance shall be for a one-year base period with four one-year renewal options. NO SOLICITATION PACKAGE EXISTS AT THIS TIME.
