Forrest, Contract Specialist (503) 326-3718 Update Diabetes Material: The contractor shall update and produce diabetes self-management education materials in booklet form. Line Item (#1), quantity 12 months, contractor shall modify, design, layout of text and graphics to produce the booklets, and be consistent with Indian Health Service (IHS) diabetes booklet series (4/99 edition). The contractor shall use text, artwork, and photographs acceptable to American Indian communities. The contractor shall work with Portland Area IHS Diabetes Program (PADP), and IHS National Diabetes Program in identifying the booklet production schedule. Contractor shall make modifications prior to submitting a camera ready format (photos scanned for placement only) for final printing. Line Item (#2), quantity 12 months, contractor shall design and format two magazines on healthy lifestyle choices of approximately 44-60 pages each. Contractor shall be responsible for securing all necessary releases for all photo work and interviews. Contractor shall submit all materials (film negatives, photos, interview notes, story drafts, and interview releases developed for this project to the IHS National Diabetes Program at the close of the contract. IHS Diabetes staff will edit all stories for diabetes and scientific accuracy. Contractor shall be responsible for writing stories, photography, validating accuracy of final camera ready (with photos scanned for placement only). Line Item (#3), quantity 12 months, includes film, development, color copies and line art necessary to material development. Line Item (#4), PADP and IHS National Diabetes program will provide the contractor with IHS regional network contact list. The contractor shall interview and photograph individuals and families in five regions. PADP will arrange the site visits to the regions. Line Item (#5), quantity 12 months, includes postage and overnight courier service, voice and facsimile phone service. Line Item (#6), quantity 12 months, contractor shall provide a final copy edit to include camera ready (with photos scanned for placement only). This solicitation is set-aside for small business concerns as defined by Federal Acquisition Regulations. Anticipated contract will be firm-fixed price contractor for non-personal services.
