a combined synopsis/solicitation for commercial items prepared in
accordance with the Federal Acquisition Regulation (FAR) sub part
12.6., as supplemented with additional information included in this
notice. This announcement constitutes the only solicitation for
proposals; a written solicitation WILL NOT be issued. Solicitation
number 797-FDF3-00-0024 is issued as a Request for Quotation The
solicitation document and incorporated provisions and clauses are those
in effect through FAC 90-46. The SIC code is 3844 and the small
business size standard is 500 employees. The contractor shall provide
the following: Quantity of 1, ORCA Mobile Fluoroscopic C-Arm Unit,
Quantity of 1, Hard Drive Storage Device which stores up to 6,000
images for review and printing; Quantity of 1, Infrared Remote Controle
Package (to include drapes; Quantity of 1, Sony UP-960 High Resolution
B&W Video Printer, Quantity of 2 Users Manuals. The contractor shall
provide delivery TO 42 MDSS/SGSLA, 330 Kirkpatrick AvenueEast, Maxwell
AFB, AL 36112-6219. . Responsible offerors shall provide the
information delineated in FAR clause 52.212-1, Instructions to Offerors
Commercial Items and a completed copy of provision 52.212-3, Offeror
Representations and Certification -- Commercial Items. Offerors shall
also provide Commercial and Government Entity (CAGE) Code, Contractor
Establishment Code (DUNS), and Tax Identification Number. Clause
52.212-2, Evaluation -- Commerical Items, applies to this acquisition.
The Government intends to evaluate offers and award a contract without
discussions. Award will be made to the responsible offeror whose offer
is most advantageous to the Government based on the following factors:
(1) Technical Capability, (2) Past Performance, and (3) Price.
Technical capability shall include, but is not limited to compatibility
with the aforementioned existing items and timeliness of performance
(best delivery date). Past performance shall include, but is not
limited to experience with similar contracts with the Government of
similar magnitude and acceptable performance. Offerors shall provide
technical information and a minimum of three past performance
references, preferably government references. Technical and past
performance when combined are more important than price. The resulting
award will incorporate the requirements of the clause at FAR 52.212-4,
Contract Terms and Conditions -- Commercial Items. Clause 52.212-5,
Contract Terms and Conditions Required to Implement Statutes or
Executive Orders -- Commercial Items, applies to this acquisition.
