Indian Hospital, Cherokee, North Carolina, weekly if required on
Tuesday mornings at a time set-up by the Government's Medical Director
and coordinated with the Contractor to provide on-site services to
diagnose cardiac diseases, valvular lesions,arterial doppler studies on
legs and carotid occlusive diseases. (Offeror to submit a bid on per
patient as unit price with total amount extended. Offeror to invoice on
per patient examined. Offeror price must include total remuneration
afforded the Contractor by the Government. No other reimbursement, such
as trasportation, insurance, food, lodging, etc., will be authorized.)
2.) the person(s) performing under this contract shall be a Board
Certified Internist for reading the studies and a Registered
Ultrasonographer for obtaining the ultrasound images at the Cherokee
Indian Hospital. 3.) The Contractor will provide all equipment to
perform studies. The Government will provide a work area for Contractor
to obtain ultrasound images in a professional and efficient manner. 4.)
Preliminary reports will be given to Government before the Contractor
leaves the Cherokee Indian Hospital. 5.) A typed diagnostic report
containing test procedure, patient name, patient's IHS chart number,
Government attending physician, results, diagnosis, interpretation of
test, and signature of reading physician shall be the format. The
reports shall be mailed to the Cherokee Indian Hospital within two (2)
work days after study to the Chief of Medical Staff. Any
interpretations considered life or death for the patient shall be
called into the referring Government physician. 6.) No change in the
personnel actually performing under this Contract will be permissible
without prior approval of the Medical Director of the Cherokee Indian
Hospital or his/her designee. 7.) Personnel Qualification- (a.) For
reading of studies: Licensure- a valid license to practice medicine in
the State of North Carolina is required. (b.) Required Training in
Medical Specialty- board certification in Internal Medicine and
Cardiology. (c.) Credentials- Bidder must submit copies of credentials
on Board Certified Internist(s) and for Registered Technologist(s).
