THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IAW THE FORMAT IN FAR SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. This notice RFQ # DTRA01-99-T-0011, constitutes a Request for Quotation (RFQ) for the following: Installation of 14 mil Shatter Resistant Window Film. The contractor shall install, strictly in accordance with Manufacturer's written installation instructions (available upon request), 27,274.58 square feet of MADICO LCL 1400 XSR, Shatter Resistant Window Film. Installation will cover the inside of all exterior windows and storefronts (13,306.16/sf) at the DTRA HQ (GTE) Building located at 45045 Aviation Drive, Dulles, VA, and the DTRA Dulles West Building (13,968.42/sf) located at 44965 Aviation Drive, Dulles, VA. Installation at the Dulles West Building will include all DTRA occupied space, as well as other tenant's space. ACTUAL INSTALLATION CAN NOT TAKE PLACE UNTIL GSA COMPLETES THE SUPPLEMENTAL LEASE AGREEMENT (SLA) AND DTRA RECEIVES AIRPORT AUTHORITY APPROVAL. Delivery will be required within 30 days from receipt of SLA and subsequent Airport Authority approval. The Contractor must have a Virginia Class A Contractor's license and insurance coverage (copies of which shall be provided). THIS NOTICE SERVES AS THE ONLY SOLICITATION; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. This procurement is being conducted under the Simplified Acquisition Threshold (SAT) portion of the Federal Acquisition Streamlining Act. This Solicitation document has incorporated provisions and clauses that are in effect through Federal Acquisition Circular 97-13. The procedures identified in FAR 13.106 apply to this acquisition. Award shall be made on a best value basis, price and past performance considered. The Government reserves the right to award to other than the lowest priced offeror. Each offeror will be evaluated based upon his/her performance under existing/prior contracts, within the past year, for project(s) of 500 square feet or more, where a 14 mil or thicker film has been installed. Offerors must supply a project name and/or contract number, name of contracting activity, description of contract scope (total square feet of 14 mil or thicker installation), dollar value, and a point of contact name and phone number for verification. Quotations shall be priced on a "price per square foot" basis. The following FAR Clauses apply to this acquisition: 52.212-1, Instructions to Offerors-Commercial; 52.212-3, Offeror Representations and Certifications-Commercial Items (include a completed copy of this provision with your offer); 52.212-4, Contract Terms and Conditions -- Commercial Items; 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items with 52.222-26, Equal Opportunity, 52.222-35, Affirmative Action for Special Disabled Veterans and Veterans of the Vietnam Era, and 52.232-33, Mandatory Information For Electronic Funds Transfer Payment; 52.217-6, Option to Increase Quantity, and 52.228-5, Insurance-Work On A Government Installation with the following minimum limits: Workmen's Compensation and Employers Liability Insurance -- $100,000; General Liability Insurance -- $500,000 per occurrence; and Automobile Liability Insurance -- $200,000 per person, $500,000 per occurrence for bodily injury, and $20,000 per occurrence for property damage.
