R -- COURIER SEVICE SOL RFQ 673-21-00 DUE 091399 POC Mick Dubuis 8123-972-7593 This is a combined synopsis/solicitation for commercial items prepared in accordance with the subparts in Federal Acquisition Regulation 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; bids are being requested and a written solicitation will not be issued. Solicitation number Request For Proposal (RFQ) 673-21-00 is assigned and shall be referenced on any proposals. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-13. The Standard Industrial Classification Code is 4215 and the business size standard is 18.5 million. This is a small business set aside. The James A. Haley Veterans Hospital, located at 13000 Bruce B. Downs Blvd, Tampa, Florida 333612 has a requirement for courier service to and from it's Brevard Outpatient Clinic (OPC) locate at 2900 Veterans Way, Melbourne, Florida 32940. The successful offeror shall provide courier service for the Brevard VA OPC, Monday through Friday excluding federal holidays, for the period of October 1, 1999 through September 30, 2000. This contract shall have four 12 month options, running from October 1, of the subsequent years through September 30, of the following year. Courier service is to originate in Tampa with a pick up at the James A. Haley Veterans Hospital warehouse. Driver is to pick up facility mail, pharmaceuticals, and other materials that will vary from day to day. The courier will deliver items to the East Central Florida Brevard OPC before 4:00 PM. And pick up diagnostic samples, facility mail and other materials and depart the clinic no earlier than 4:00 PM for delivery back to the James A. Haley Veterans Hospital. Proposals shall be submitted on an "all or none basis." Partial quotations shall be rejected. All potential offerors shall be available to perform the requirements of this contract effective October 1, 1999. Offerors shall provide a complete set of provisions with proposal found at FAR52.212-3, Offeror Representation and Certifications-Commercial Items.
