65 -- NOTICE OF INTENT TO SOLE SOURCE SOL 69D-398-99 DUE 083099 POC Diane Brazale, Contracting Officer (414) 902-5403 The Great Lakes Acquisition Center intends to negotiate a Sole Source Contract with the Tracer Corporation of 8177 Glades Rd. Boca Raton, FL for purchase of two (2) TracerCad Systems for the VA Hospital, Fifth & Roosevelt Rd. Hines, IL 60141. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Far Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Solicitation number RFQ 69D-398-99 has been assigned to this procurement. Solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-13. SIC (3842). Vendor shall provide all labor, material, tools and equipment necessary to furnish and install (2) two TracerCad Systems. Equipment shall be shipped FOB Destination to the attention of the Chief, Prosthetics, VA Hospital Fifth & Roosevelt, Hines IL 60141 and delivered within 30 calendar days from receipt of Purchase Order. The equipment MUST be compatible with the existing systems at the hospital, be able to function simultaneously with the existing CNC Milling Machine and enable hospital personnel to carve a fully formed AFO. Vendor shall provide training to hospital personnel at a mutually agreed upon time with the Chief, Prosthetics and the CO. Cost for same is included in the contract price. This will be a firm fixed price contract AND must include all costs associated with the delivery, training and installation of the equipment. The TracerCad requires the use of proprietary software. License agreement is included. Vendor shall provide a full one-year warranty on all parts and labor, software updates and technical support from the date of final acceptance by the Government. Payment will be made in arrears on certified invoice upon final acceptance by the Government. All equipment, replacement parts, components, hardware, software and firmware containing information technology delivered under this contract shall be Year 2000 Compliant. Year 2000 Compliance means that information technology accurately processes date/time data (including, but not limited to, calculating, comparing, and sequencing) from, into, and between the twentieth and twenty-first centuries, and the years 1999 and 2000 and leap year calculations. The Vendor is required, as part of the offer, to provide documentary evidence that equipment, replacement parts, components, hardware, software and firmware offered is Year 2000 Compliant. The proposed contract action is for the purchase and installation of this equipment, which the Government intends to solicit and negotiate with only one source under the authority of FAR 6.302-1. All FAR provisions and clauses may be reviewed and/or obtained from the General Service Administration's Federal Acquisition Regulation website at Internet address http://www.arnet.gov/far. This notice of intent is not a request for competitive proposals. A determination by the Government not to compete this proposed contract, based upon responses to this notice, is solely with the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement.
