Box 19888, 236 Goodwin Crest Drive, Homewood, AL 35219-0888. This procurement shall be processed in accordance with the Simplified Acquisition Procedures of Part 13 of the Federal Acquisitions Regulations. The purpose of this purchase order is to prepare a prospectus for State Agencies serving persons who have experienced a Traumatic Brain Injury (TBI), and provide information to be made available for decision makers regarding services for TBI individuals. The contractor shall provide an outreach of TBI information to decision makers in state governments, including Maternal and Child Health, Emergency Medical Services, Rehabilitation Services, Medicaid, and Mental Health Services. The contractor shall develop and maintain information such as annual reports, fact sheets on TBI, a website on TBI, and quarterly reports on special TBI topics. The period of performance for this project is one (1) year. The National Association of State Head Injury Administrators is deemed the only responsible source capable of providing required work and services.
