Provide readjustment counseling services for eligible Vietnam Era Veterans and Veterans of the Persian Gulf War and Conflicts in Lebanon, Grenada, Panama and Somalia that are referred by the VA Readjustment Counseling Vet Center, Louisville, Kentucky. Contract is for the period of October 1, 1999 through September 30, 2000 with two (2) one year options, pending availability of funds. Geographic coverage for this service will be the following counties of Kentucky: Adair, Breckinridge, Bullitt, Carroll, Casey, Clinton, Davies, Edmonson, Grayson, Green, Hancock, Hardin, Hart, Henry, Larue, Marion, McLean, Meade, Metcalfe, Nelson, Ohio, Oldham, Owen, Russell, Shelby, Spenser, Taylor, Washington, and Trimble. Contractor will be required to furnish approved therapy programs for set fees. Service providers submitting offers in response to the request for proposal must be physically located where a Vet Center is not present or in counties that are being serviced by a Vet Center and must be established, licensed business, with a proven track record of providing such services.
