for quotation for a commercial item prepared in accordance with the
format in FAR Subpart 12.6, as supplemented with drawings, wiring
schematic and acceptable material listing to be provided upon written
request. This announcement constitutes the only request for quotation;
proposals are being requested and a written request for proposals will
not be issued. This request for quotation incorporates provisions and
clauses in effect through Federal Acquisition Circular 97-12. This is
a Small Business Set-Aside procurement. Contract award will be
contingent upon full acceptance of a First Article to be evaluated by
a government agent at Aircraft Repair & Supply Center, Elizabeth City,
NC. The U.S. Coast Guard at Elizabeth City, NC requires the
fabrication/manufacturing of Ten (10) each HH60-J Auxiliary Tank Fuel
Probe Test boxes. The test boxes are to be of water-tight construction
IAW Mil-Std-108 requirements; color to be yellow gloss enamel. Test
box dimensions to be approximately Thirteen (13) inches in length,
Eleven (11) inches in width and Ten (10) inches in height. The cable
lengths are to be: M2 cable assembly Five (5) feet from end of
connector to test box; M1 cable assembly Twelve (12) feet from end of
connector to test box. The cable assemblies (M1 and M2) are to be
detachable from test box. The test boxes are to be assembled utilizing
the material listing, electrical connections, with each item
identified per wiring schematic. Proposal of design and layout of
components will be evaluated for compatibility with the 120 gallon and
80 gallon auxiliary tank components to be tested. The Standard
Industrial Classification Code is 3728. The small business size
standard is 1000 employees. Items are to be delivered to USCG Aircraft
Repair & Supply Center, Receiving section, Bldg. 63, Elizabeth City,
NC 27909. AR & SC Receiving section will perform inspection and
acceptance. Closing date and time for receipt of offers is 3:00 p.m.,
August 20, 1999.Facsimile offers are acceptable and may be forwarded
via fax number (252)335-6790. Offerors are instructed to include a
completed copy of FAR provision 52.212-3 "Offeror Representations and
Certifications Commercial Items", with their bid. If a copy of this
certification is needed, it may be requested by calling the contact
number identified above. Offers may be submitted on company letterhead
stationary indicating the nomenclature, part number, unit price and
extended price for each item; plus the FOB point, and any discounts
offered for prompt payment; the business size standard and any minority
classification. A company representative authorized to commit the
company must sign the offer. Provision/clause FAR 52.209-04 applies to
this request for quotation and reads as follows: FIRST ARTICLE
APPROVAL GOVERNMENT TESTING. (a) The Contractor shall deliver one (1)
Auxiliary Tank Fuel Probe Test Box within 90 calendar days from the
date of this contract to the Government at ARSC HH-60J Engineering Cell
for first article test. (b) Within fourteen days after the Government
receives the first article, the Contracting Officer shall notify the
Contractor, in writing, of the conditional approval, approval, or
disapproval of the first article. The following FAR provisions/clauses
