The USDA, Agricultural Research Service, Midwest Area, Peoria, IL has the requirement for Employee Assistance Program (EAP) Services. Solicitation No. 5114-9-3132 is being issued as a Request for Quotation. The contract period will be from October 1, 1999 through September 30, 2000, with four one-year option periods. The contract will be for appr9oximately 425-475 employees in the following areas: Peoria, Illinois; Madison, Wisconsin; Morris, Minnesota; and Columbia, Missouri. Contract performance will include (1) acceptance of management, union, family, or self-referrals where professional counseling is necessary; (2) evaluation of existing problems; (3) individual short term counseling (one -- three sessions); (4) referral to appropriate community treatment resources; (5) monitoring of employee progress; (6) management consultation as requested; (7) training of agency officials and employees; (8) statistical reports. The range of problems to be covered includes any mental/emotional/behavioral problem, especially those related to alcohol/drug use, which may adversely impact job performance or behavior.
