65 -- PRESCRIPTION EYEGLASS SERVICES SOL RFP-642-13-99 DUE 091799 POC Contract Specialist, Greta Turner, (215-823-5174) The contract shall furnish Prescription Eyeglass Services to eligible veterans of the VA Stars & Stripes Healthcare Network (VISN 4). The contractor will be responsible for providing service in regularly established facilities located in easily accessible areas, well-dispersed throughout a geographical services area. VISN 4 services areas comprise Pennsylvania, Southern New Jersey, Wilmington, DE and Clarksburg, WV. All eye examinations and prescriptions will be furnished by the VA. Contract performance shall include the fabrication and delivery of prescription eyeglasses to veteran beneficiaries. Veterans will have the option of having the eyeglass prescription filled and delivered to their homes or to bring the prescription to an authorized contractor facility for fabrication, fitting and direct receipt of the eyelasses. The term of this contract is for one year from date of award, with two (2) one year options. VA anticipates award of multiple contracts from this solicitation and will award a contract to responsible offerors whose offer confirming to the solicitation will be most advantageous to the Government, price and other factors considered. SIC code for this procurement is 5995. This solicitation adheres to the format defined in FAR Part 12 for the acquisition of commercial items and includes small disadvantaged business (SDB) participation criteria.
